- This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 ? THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL. VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ/RFP IS POSTED TO THIS WEBSITE. 45 CONS/LGCB has a requirement for CEREC Inlab Dental Laboratory CAD/CAM System (Item #: 5884742) to include the following: InEos Dental Digitizer Scanner (Item #: 6005909), Milling Activation Key (500 Units), CAD/CAM Computer system to be set-up and calibrated (purchase price to include all shipping/handling costs, the training course fee and starter supplies of ceramic materials delivered at the completion of the training). This is a Brand Name requirement ? ?or equal? items will not be accepted. This equipment will be delivered to Patrick AFB, FL. Period of performance/expected delivery date is thirty (30) days after receipt of an order. The government intends to make a single award to the lowest priced, technically acceptable offeror. This proposed procurement is a 100% small business set-aside. Solicitation No. FA2521-06-T-0050 is issued as a Request for Quotation. It is advertised with a Standard Industrial Code (SIC) of 3843, business size of 500 employees and a North American Industry Classification System (NAICS) code of 339114. GSA Schedule holders for this item/service are encouraged to provide us with their GSA Schedule number. All responsible sources may submit a proposal, which shall be considered by the agency. The solicitation will be issued electronically on or about 30 January 2006 at www.fbo.gov . Proposals may be submitted via hard copy, fax (321-494-5136) or via e-mail.
