- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05. All applicable provision and clauses are included in the actual solicitation. This procurement is set for a 100% small business set aside. The North American Industry Classification System (NAICS) code is 339112, and the small business size standard is 500 employees. The Government intends to issue a firm fixed price contract with 2 option years. STATEMENT OF WORK 55 WG/HC CATHOLIC COMMUNITY COORDINATOR 1. SERVICES NON-PERSONAL: To provide catholic community coordinator duties for the Offutt Chapel starting on or around 7 Feb 06 until 30 Sep 06. 2. PERFORMANCE REQUIREMENTS: a. Individual must work closely with the Catholic Chaplain, be proficient in Microsoft Word, Excel, PowerPoint, Microsoft Publisher, and Access. b. Developing and coordinating the various processes for servicing needs within the Catholic Community Chapel Program including conducting needs assessment and satisfaction surveys at least once a year, compiling and filing survey results; coordinating infant baptism preparation for parents, coordinate marriage preparation process by directing couples to Senior Catholic Chaplain or his designated facilitator for this program; maintain liaison with the Archdiocese of Omaha Family Life Office and Youth Office to resource their services; oversee the activities of the total parish community. c. Plans and coordinates all Catholic program information for parish and base publications as determined by the Catholic Chaplain and for integration with the entire Base Chapel Program. This would include typing and preparing for printing the weekly Catholic Sunday bulletins and other special/seasonal bulletins as needed throughout the year. d. Coordinates with parish organizations for all Catholic program scheduling requirements to include preparing and distributing annual monthly parish calendars, compiling and distributing every two months liturgical ministry schedules with consultation of each ministry head, coordinates and monitoring all facility requests for the year concerning Catholic programs, keeping informed and aware of any possible conflicts with other Chapel activities, and providing planning material and resources for the annual parish pastoral planning session. e. Maintains Catholic marriage preparation program folders as established by the Senior Catholic Chaplain. f. Coordinates with Accounting Technician in preparation of annual monthly budget needs and fund requests for the Catholic program to include compiling, tracking and publishing Mass attendance and offering figures. These weekly figures of Mass attendance and offering are to be included, if space permits, in the weekly Sunday Bulletin. g. Serves as the Account Manager for the Catholic Sub-Account and coordinates with the Accounting Technician in preparation of annual and monthly budget needs and fund requests for the Protestant program to include compiling, tracking and publishing worship attendance and offering figures. These weekly figures of worship attendance and offering are to be included in the weekly Sunday bulletin announcements. h. Promotes via publications and resources seasonal activities (Advent/Christmas, Lent/Easter, Holy Days), education/formation opportunities, liturgy/prayer, and publications in accordance with Catholic Church teaching and AMS guidelines. i. Coordinates the Sacramental Records registration process within the Catholic Chapel program and with the Archdiocese for the Military Services, USA. This includes preparing copies for the parishioners? file, our ?in house? file and the original for the AMS file. Each quarter there is also the Chaplain Quarterly Sacramental Record for the AMS, which is included in the above mentioned process. j. Maintains a current database of parish members and their families through parish registration, religious education registration, annual parish census update, ?Welcome Weekend,? and other outreach events and actively assisting in the recruitment/channeling of volunteers for parish organizations and groups. k. Prepares and maintains a continuity file on parish activities to include all planning material and all ?After Action Reports? of special parish events and compiles an annual historical report. l. Facilitates and coordinates communication between and among ALL Chapel programs (Liturgical Committees, Fellowship, Men?s, Women?s, Single?s, Youth, MOMS, Religious Education) as well as various outreach programs (shelters, volunteer programs, angel tree, food for the poor, clothes drives). Gives assistance to the Religious Education Coordinator. m. Ensures planning and coordination of the many social/spiritual/community enhancing activities as parish council demes necessary, such as: a) annual parish picnic; b) Halloween/All Saints celebration end of October; c) Turkey Bingo and Advent Wreath making/Christmas preparation social in November/Christmas ?carols and cookies? event in conjunction with the R.E. Coordinator; d) Winter Social in late January if parish desires one; e) St. Valentine/Sweetheart Banquet and Marriage Renewal Mass in February; f) St. Joseph/St. Patrick celebration in March; g) Lenten Soup Suppers with Stations of the Cross/Lenten devotions; h) Easter Egg Hunt in coordination with the Children?s Religious Education Program; i) May crowning of the Image of Mary in the 1st or 2nd week of May. n. Attends regularly the Parish Pastoral Advisory Council meetings and Wing Chapel staff meetings to report on Catholic Parish ministry and activities to include maintaining and updating the parish slides. Also attends all scheduled liturgy meetings on a regular basis to assist in coordination of dates, times and places of all parish liturgies (especially seasonal liturgical events, i.e. Advent, Christmas, Lent, Easter, etc.). Meetings may be conducted prior to or after normal chapel duty hours which are Monday ? Friday, 0730 ? 1630 o. Catholic Community Coordinator must ensure Quality Control of coordinator programs during regularly scheduled weekend worship liturgies and rites per annum. p. Coordinates and prepares monthly priest schedule, to include contacting local priests to fill the needs of the parish. q. Safeguards all government/Chapel equipment and supplies provided to fulfill this position. r. Provide Catholic Community Coordinator customer service hours at Capehart Chapel or at the SAC Memorial Chapel from 0900 hrs to 1500 hrs Monday through Thursday. Meetings and other engagements may also take place during regular chapel duty hours. During these hours Contractor is entitled to one hour for lunch. Contractor will notify the Senior Catholic Chaplain of any absences and, if absent, will arrange to accomplish the tasks of this Statement of Work at other times or provide a substitute of equal or higher proficiency who must be approved in advance by the Senior Catholic Chaplain. s. Prepares annual budget and monthly budget reports for the Parish Pastoral Council as well as facility requests for program materials and activities; provides monthly participation reports of Catholic Parish programs to the necessary Chapel personnel. The contractor will submit monthly invoices for services rendered in accordance with the contract to be certified by the Catholic Chaplain or designated chapel representative in the event the Catholic Chaplain is unavailable. 3. Final payment under terms of this contract will not be issued until all facility keys are returned to the appropriate personnel.
