- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This is a Request For Proposals (RFP). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation (RFP 06-3K06-007) is hereby issued as a request for proposal (RFP). A sources sought was posted for this requirement under number 05-3K06-083 and is replaced with RFP 06-3K06-007. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-08. This solicitation is being issued as full and open competition. The applicable NAICS code is 541110 and size standard for this requirement is $6 Million. All responsible sources may submit a proposal which will be considered by the agency. Amendment 1 includes: Instructions to Offerors FAR 52-212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (JAN 2004) INSTRUCTIONS FOR SUBMISSION OF PROPOSAL (JAN 2006) ? One (1) original and three (3) copies of the Technical and Business/Price Proposal, signed by an official authorized to bind the Offeror, shall be submitted via email, so as to be received not later than the time and date at the location specified in Section A above. INSTRUCTIONS FOR SUBMISION OF TECHNICAL PROPOSAL: The Offeror shall submit a technical proposal describing, at a minimum, the following: a. How the requirements of Attachment I, Statement of Work, will be accomplished. The information shall be in sufficient detail to clearly understand how the requirements of the statement of work will be met. b. The proposal shall include a detailed Quality Control Plan by which the accuracy and precision of the generated data can be determined by Appendix A-of Attachment I-Statement of Work. The discussion shall address each requirement listed in the Quality Assurance Surveillance Plan. c. The offeror?s technical proposal shall demonstrate compliance with FAR 45.505, ?Record and Reports of Government Property.? For a complete electronic version of the Federal Acquisition Regulation (FAR), please refer to www.arnet.gov/far. d. Subcontract opportunities are not envisioned for this commercial service. Offerors intending to subcontract out any portion of the SOW shall describe in detail the extent subcontractors will be utilized to meet the requirements of the SOW. INSTRUCTIONS FOR SUBMISSION OF BUSINESS/PRICE PROPOSAL: The Offeror shall submit the following : a. Two (2) copies Standard Form 1449, one original copy must be signed with original signature. Offerors shall call the Contracting Officer to request a copy of the SF 1449. b. The Offeror shall complete Section I.A of the solicitation. c. A Firm-Fixed Price for each line item in the Price Schedule in Section I.A. d. Ensure the online Offeror?s Representations and Certifications have been completed and posted. e. A list of references that can substantiate successful past performance for the same or similar work. Attachment III is a list of sample questions that will be asked your references. Please provide the following information regarding past performance: Discuss your company?s experience which is similar to the Government?s requirements in (1) services to pay annuities and taxes; (2) services to docket and track cases; (3) services to provide receipts and reports, tax maintenance lists, and confirmation reports. Discuss any special abilities of your company not covered above which relate to patent annuities. Indicate the approximate percentage of your company?s activities that is directed to patent annuities. Government experience is preferred and will receive additional consideration. List five (5) contracts of a nature and complexity similar to the requirements above awarded or performed within the past 5 years. For each contract listed, provide the following information: Name, address, email address, fax and telephone number of the contracting organization, the Project Officer, and the Contracting Officer, contract number, type, and dollar value, date of contract and period of performance, brief description of contract work, scope, and responsibilities, discuss the similarities and differences between the proposed effort and that contract, discuss any special problems encountered during the performance of services under the contracts, average number of technical personnel involved (by labor skill), percentage turnover of contract technical personnel/year, identify any contract similar to the proposed requirement which was terminated for convenience or default within the past five years. include the contract number, name, address, email address, fax and telephone number of the terminating Contracting Officer. f. Include financial statements for the last two years so that financial risk can be evaluated.
