Description

(i) This is combined synopsis/solicitation for commercial items number FMA480053470010 issued on Dec 09, 2005 for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation will be issued on the World Wide Web and can be found at the following web site: http://www.eps.gov/. (ii) Solicitation FMA480053470010 is issued as a Request for Quote using Simplified Acquisition Procedures under the test program for certain commercial items found at FAR Part 13.5. (iii) The solicitation document and FAR incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-06 and Defense Federal Acquisition Regulation Supplement, Defense Change Notice (DCN) 20051220. Special Provisions; and the Representations, Certifications and Other Statements of Offerors or Quoters are included with this solicitation as an attachment. (iv) This acquisition is a 100% Small Business Set-Aside IAW FAR 19.502-2(b). The NAICS code is 333314 and the small business size standard is 500 employees (v) The 1st Contracting Squadron has a requirement for a Surgical microscopes purchase for the Medical Logistics Squadron, 73 Nealy Ave, Langley AFB, VA 23665. The contract will be for one (1) base year. The Medical Logistics squadron will be using the necessary equipment to perform cataract and glaucoma surgery. The surgery will assists in viewing ocular structure during surgery. All items listed above are to be or equal. Delivery: Date of delivery shall be 6 weeks after date of Contract. Commercial Items (JAN 1999), (a) The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government.
