THE ABOVE REFERENCED RFQ IS MODIFIED AS FOLLOWS: ADD THE FOLLOWING: AMENDMENT ONE / RFQ 06EIOS060041 QUESTION AND ANSWERS: 1. Where will the monthly committee meetings be held (e.g.: state, city, etc.)a: They will occur by conference call from the OD office located in Washington, DC a. Have sites been selected for the meetings? Will the contractor be responsible for site selection and signing the hotel contract? N/A b. How many sleeping rooms are required? N/A c. How many representatives from each of the 78 national organizations will be attending? A: All of the 78 or a portion of the organizations will participate on the conference call meetings contingent upon agenda topic and respective availability. d. How many Federal representatives will be in attendance? A: At least 3 to 4 federal representatives participate on the conference call meetings. e. What is he duration of the meetings. A: The typical conference call is up to 1.5 hours. 2. Are the committee members responsible for arranging their travel and lodging accommodations and ground transportation arrangements? A: Local organizations may participate in person; otherwise the Office on Disability provides the conference call in capacity. 3. Will the contractor be responsible foe providing expenses reimbursements for committee members? N/A 4. Will a website be developed by the contractor for meeting registration? N/A 5. Will the committee members receive honoraria? If so, how much? N/A 6. Does a listserv currently exist to maintain communication? A: yes 7. Will contractor be responsible for agenda and all meeting materials? A: Yes 8. WHO IS THE INCUMBENT: Michael Marge , is a follow-on purchase order.
