- THIS IS NOT A SYNOPSIS FOR A REQUEST FOR PROPOSAL. It is a market research tool being used to determine potential sources prior to the issuance of a Request for Proposal for suppliers of mortuary services. The Government is not obligated to and wil l not pay for any information received from potential sources as a result of this Sources Sought. The Contracting Center of Excellence (CCE) is soliciting information from potential sources and seeking qualified business sources on behalf of the Army Huma n Resources Command, Casualty and Memorial Affairs Operations Center to provide all supplies and services necessary for the care and disposition of remains of active duty military personnel within the Continental United States. Supplies and services will include: removal from place of death; appropriate preparation (sanitation, restoration and preservation) of the remains, dressing (with unionalls), casketing and shipping remains world-wide. Contractor shall maintain strict compliance with specifications and provisions outlined in Army Regulation 638-2 (Care and Disposition of Remains and Disposition of Personal Effects) and Department of the Army Pamphlet 638-2 (Procedures for the Care and Disposition of Remains and Disposition of Personal Effects). Cont ractor shall be required to certify all supplies and services meet specifications in Block 13 of Department of Defense (DD) Form 2063 (Record of Preparation and Disposition of Remains). A sample completed DD Form 2063 is located at Figure 5-1 (page 24) De partment of the Army Pamphlet 638-2. Contractor shall be available to receive requests 24 hours per day/7 days a week/365 days per year. Preparation must be complete and remains ready to ship within 36 hours from the time request is made. The North Ame rican Industry Classification System (NAICS) code is 812210. Respondents having the skills and capabilities necessary to perform the stated requirements are invited to provide information to contribute to this market survey/sources sought including 1) Tech nical capabilities; 2) Personnel resumes which include related work experience and; 3) Company experience/qualifications of comparable mortuary services. Information provided by the respondents in response to this Market Survey shall not be disclosed if doing so would jeopardize the respondents confidential information and shall be protected under FAR Part 3.104.
