Description

This document sets forth the requirements for the professional services contract that meets the following objectives and must be pre-approved by DRA: The successful proposer will be responsible for development of a region-wide diabetes media outreach plan that will leverage media coverage at least 1:1 of equal value. The successful proposer will be responsible for obtaining additional funding from other sources besides DRA for the diabetes program. A list of contacts, whether they provided funding or not, should be maintained and available at request of DRA. The successful proposer will be responsible for the creation and coordination of public relation activities. Proposers should provide a detailed description of public relations activities that will support the program?s diabetes message. The successful proposer will be responsible for the production of all supporting materials associated with the program, including printed materials, website and broadcast commercials. The successful proposer will have experience with previous response-oriented, outcomes-based media outreach programs and be responsible for tracking response to media in the proposed program. The successful proposer will have experience with diabetes outreach and education, including media outreach for such a program. The successful proposer will create a call center which will screen callers for their need of referral for health reasons and Medicaid eligibility and then refer and/or enroll callers as appropriate. The successful proposer will create a database (to be owned by DRA) from these calls to track referral outcomes (patient progress) and Medicaid billings. Those outcomes will be reported to DRA.
