- A sole source procurement is contemplated to Cecil G. Sheps Center for Health Services Research of North Carolina to provide regular updates to a software system that identifies physicians located in a Health Professional Shortage Area (HPSA) who are eligible for HPSA bonus payments. Section 413(a) and 413(b) of the Medicare Prescription Drug and Modernization Act (MMA) of 2003 require revisions to the current HPSA bonus process. Currently, Medicare carriers pay the HPSA bonus based on the inclusion of a modifier with the service when it is billed. Designations of the HPSA bonus areas are based on determinations of primary care and mental health rural and urban whole and part county geographic HPSAs made by The Health Resources and Services Administration (HRSA). These designations may be by county, census tract, township, etc. Based on the legislation, that policy will be revised to no longer require the modifier for services rendered in zip code areas that fully fall within a designated HPSA area or are classified as dominant according to the United States Postal Service (USPS). Services provided in areas that are eligible for the bonus, but do not fully fall within a HPSA bonus area must still be submitted with the modifier. In order to pay the physician bonus The Centers for Medicare & Medicaid Services (CMS) must provide to the Medicare carriers and standard systems lists of the zip codes for which the bonuses may be paid. Under an existing contract, Cecil G. Sheps Center for Health Services Research developed a product to identify zip codes for which physicians will automatically be eligible for bonus payments. CMS intends to issue a sole source contract for the continued support of this system. Under this contract, Cecil G. Sheps Center for Health Services Research will be required to provide regular updates to the product that can easily be utilized by the Medicare standard systems and carriers and also be added to the CMS website, as well as develop a process to test the zip code files to verify that the zip codes are correctly linked to the geographic areas. It is imperative that CMS have a contractor that has total knowledge of the development of HPSAs as well as the geography of both HPSAs and zip code areas, experience with the relational file structure of the HPSA designations and corresponding Geographic Information System (GIS) files, how each is defined, and how each changes over time. This contract will be awarded for a period of 1 year with 4 option years. This is a notice of intent and not a request for proposals. Businesses that recognize and can provide the required service described above must provide supporting evidence. The Government will consider all responses received within 15 days of the date of publication of this synopsis. A determination by the Government not to compete this proposed acquisition based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
