- Solicitation for the possible replacement of Patient Electrodes for TRICARE Puget Sound and Alaska. These products include but are not limited to a complete product line used in patient monitoring. Those companies interested in a possible Regional Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Puget Sound and Alaska must respond to the following questions: Patient Electrodes Criteria Clinical Criteria 1. Explain how your product is easy to use and meets the following: a. Electrode application requires minimal training. b. The electrode backing is easy to remove. c. When the electrode is on the patients skin, a minimal amount of pressure is required to connect the lead to the electrode d. The electrode can be easily removed from the package. 2. Do your electrodes come in resealable bag (so that gel on the electrodes remaining in the package does not dry out)? 3. How is your product packaged to reduce storage space? 4. How does your electrode adhere to the skin? Is special skin preparation necessary? Do you have any studies that document adhesion to the skin? 5. Provide evidence that the size of the electrode is appropriate for standard monitoring and 12-lead ECG. 6. Does the electrode require the use of creams or gels? 7. Is the electrode easily removed from the skin. 8. Provide evidence that the electrode is not irritating/damaging to the skin. 9. Will your electrode stay in place when exposed to moisture (perspiration)? 10. When lead is applied to the electrode, provide evidence that the lead is not easily dislodged. 11. Will your electrode interfere with radiological studies or is it radiolucent? Technical Criteria 1. Do your electrodes meet federal standards? (FDA and AAMI)? 2. Does your company manufacture and/or distribute electrodes? 3. What brands of electrodes does your company manufacture and/or distribute? 4. How are your electrodes packaged (i.e., single package, in strips, in zip lock bag)? 5. What types of conductive materials do your electrodes contain? 6. Can your electrodes be used with any manufacturers equipment? 7. Can your electrodes be used for both short and long term applications? 8. Are your electrodes latex free and hypoallergenic? 9. Can your electrodes be used for adult, pediatric, and neonatal populations? 10. Do your electrodes require any skin preparation? If so, what kind of skin prep is needed? 11. Do your electrodes require any creams or gels for operation? 12. Does your company have a distribution agreement with Cardinal Healthcare? 13. Does your company currently possess, or in the process of obtaining, a Distribution and Pricing Agreement (DAPA)? 14. Does your company have the ability to service to the entire TRICARE Puget Sound and Alaska Region (WA and Alaska)? Please provide your companys distribution network. 15. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon reques t after product implementation throughout the length of a possible agreement? 16. Describe your companys return goods policy. 17. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska. 18. Does your company provide volume discounts? 19. Provide a history of back orders and/or recalls for this product group with dates, duration, cause, and how information was communicated to the customer, interim measures and final resolution. 20. Does your company have a 1-800 # for customer questions? 21.
