- The contractor shall furnish professional medical services, all travel, and incidentals, except as stated herein, necessary to provide medical services to students of the Pine Knot Civilian Conservation Center, Daniel Boone National Forest, Pine Knot, KY. Duties will include but will not be limited to, assisting in the development of a self health care program, orientation of new students, entrance physical exams, consultation with Center Director, and coordination with other Center Health Care Specialists. This contract is a non-personal health care services contract under which the contractor is an independent contractor. The Contracting Physician must be licensed in the Commonwealth of Kentucky and provide current copies of both Medical License and Drug Enforcement Agency (DEA) number. The Doctor must also sign an Ethics/Conduct statement.
