Description
The VA Medical Center, 1 VA Center, Augusta, Maine is seeking potential sources to provide AMBULANCE AND WHEELCHAIR VAN SERVICES. Contractor shall furnish all labor, materials, supplies, equipment and qualified personnel to provide Basic Life Support (BLS), Advanced Life Support (ALS) Ambulance and Wheelchair Van Services to veteran beneficiaries. Services to be for the entire State of Maine, twenty-four hours per day/seven days per week, nights and holidays. Service to include transport to and from VAMC Togus as well as Primary Care Clinics in Caribou, Machias, Calais, Bangor, Rumford, Portland Mental Health Clinic and Saco, as well as ambulance runs to VA Medical Centers in Massachusetts. ALL SERVICES PROVIDED AND ALL VEHICLES USED IN PERFORMANCE OF THIS CONTRACT SHALL BE IN ACCORDANCE WITH STATE OF MAINE EMERGENCY MEDICAL SERVICES (refer to http://www.maine.gov/dps/ems). Contractor shall be licensed in the State of Maine. Critical Care Transport by Ground shall also be included. Annual estimated amount: 310 BLS trips within city limits/15,000 miles beyond city limits; 310 ALS trips within city limits/7,950 miles beyond city limits. Contractor shall also provide oxygen, heart monitor, IV therapy, Infusion Pump, External Pacemaker and Defibrillator when ordered and required. Wheelchair Van: Estimated 95 trips annually. The applicable NAICS Code is 621910 with a size standard of $6 Million. Contract period will be one year from date of award with two available one-year option periods. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider; business size; socio-economic category, if any; if services are on a Federal Supply Schedule; firm?s experience in providing Ambulance and Wheelchair Van Services and any other pertinent information which demonstrates firm?s ability to meet the above requirement, in strict accordance with Maine EMS rules and regulations.
