Description
Luke AFB, AZ, has a requirement for repair of two Simon aerial lifts. The NAICS code is 811310, $6.5 mil. Below is the Statement of Work, (SOW): 1. SCOPE: The work covered by this statement of work consists of furnishing all plant, labor, equipment, appliances and materials and performing all operations in connection with the complete repair and reconditioning of 2 Simon Aerial Lifts. Note: Work may be performed on site, (Luke AFB, AZ), or at the contractor?s site. 2. LIFT 1 ? Model 32/21P, Serial # DAA01110, Mfg. date 12/1994 A. Transport lift inbound and outbound. B. Repair major hydraulic oil leak at hose assemblies from control valve to steering cylinder by replacing hoses. C. Replace worn protective wrap around hydraulic hose assemblies as required. D. Replace faulty warning alarm unit at platform. E. Repair engine starting and running problem by: replace dead battery, corroded battery cables and faulty battery disconnect switch. Replace faulty ignition switch at platform station. Replace burnt Ground/Platform controls selector switch. F. Tune up engine by rebuilding carburetor unit. Replace sparkplugs and ignition wires. Drain and discard old gasoline and fill with 5 gallons of fresh fuel. Adjust engine to specifications. G. Perform periodic maintenance by replacing air filters. Replace oil and oil filter element. Replace fuel filter. Replace hydraulic fluid and hydraulic filters. Check and fill all fluid levels. Lubricate all pivot points and grease zerk fittings. Lubricate boom extension sections wear pads. Lubricate rotation bearing raceways and gear teeth. H. Supply customer with and place in unit copy of operator instructions and safety manuals. I. Replace Platform control station functions label decal. J. Test all control functions and safety devices. 3. LIFT 2 ? Model 32/21G, Serial # DAA01152, Mfg. date 01/1997 A. Transport lift inbound and outbound. B. Replace worn hydraulic drive coupling and pump. C. Replace worn protective wrap around hydraulic hose assemblies as required. D. Repair engine starting and running problem by replacing dead battery and corroded battery cables. E. Tune up engine by rebuilding carburetor unit. Replace sparkplugs and ignition wires. Drain and discard old gasoline and fill with 5 gallons of fresh fuel. Adjust engine to specifications. F. Perform periodic maintenance by replacing air filters. Replace oil and oil filter element. Replace fuel filter. Replace hydraulic fluid and hydraulic filters. Check and fill all fluid levels. Lubricate all pivot points and grease zerk fittings. Lubricate boom extension sections wear pads. Lubricate rotation bearing raceways and gear teeth. G. Replace faulty engine Hi/Low speed control solenoid. H. Supply customer with and place in unit copy of operator instructions and safety manuals. I. Test all control functions and safety devices. 4. WARRANTY: The contractor shall provide a warranty for 90 days or 600 hours of operation which ever is first. 5. PERFORMANCE PERIOD: Work under this contract shall be completed within 60 calendar days after receipt of purchase order. 6. SITE VISIT: One site visit is going to be held on 23 March 2006, Thursday, 9:30 AM, Mountain Standard Time, at 56 CES, CEOI, Bldg 339, 13970 W Lightening St, Luke AFB, AZ 85309.
