- 01-General The Contractor shall provide all labor, material, and equipment necessary to perform the Safety inspection and testing for the Annual, Routine, and the (5) five year weight and load test in accordance with the applicable codes and requirements at the following locations: William Green FOB 600 Arch Street Phila., Pa. 19106 James Byrne Court House 601 Market Street Phila., Pa. 19106 US Custom House 2nd & Chestnut Streets Phila., Pa. 19106 Robert Nix FOB 9th & Market Street Phila., Pa. 19107 The Contract will be for a (5) five year period, (1) one base year and (4) option years. Each year in accordance with the bid sheet the elevator testing will indicate a five year inspection or an annual inspection. The Contractor shall provide a price as per the attached bid sheet. 02- Safety and Health A- All work shall comply with the applicable requirements of 29CFR 1910/ 1926 AND 40CFR 761. All work shall comply with applicable federal, state, and municipal safety and health requirements. Where there is a conflict between applicable regulations, the most stringent will apply. B- The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of the work, and shall hold the Government harmless for any action on its part or that of its employees or subcontractors, which results in illness or death. C- The Contractor shall provide all necessary safety equipment and ensure that such equipment and all relevant safety procedures are adequate for the job being performed and are utilized properly. 03- Working Schedule A- All testing can be preformed during working hours Monday through Friday from 6:00am to 6:00pm. Except performing the (5) five year test for the following elevators must be inspected after hours Monday through Friday 6:00pm to 6:00am and anytime on Holidays, Saturday, and Sundays. James Byrne Court House 1- Judges Elevators 2- Prisoner Elevators 3- Byrne Freight Elevator. Robert Nix Federal Building and Court House 1 ? Judge elevator (one each) 2 ? Prisoner Elevator (one each) B- All work shall be scheduled and coordinated with the existing maintenance Elevator Contractor and the GSA Representative. The base year of the contract the Robert Nix Federal Building should be tested after May 2006. The option years should be tested as scheduled, when the other buildings are tested. C- The base time for completion shall be 45 days of receipt of notice to proceed. All option years should be completed 45 days of receipt of notice that the option was exercised. D? There may not be an excessive number of elevators out of service due to the Testing of the elevators. The following are the amount of elevators that may be out of service at one time performing the test. A? William Green FOB, two elevators at a time. One elevator per elevator bank can be out of service during the test. B ? James Byrne Court House, one elevator at a time. C - US Custom House, one elevator at a time. D ? Robert Nix Federal Building and Court House, one elevator at a time. 04- Additional Testing Requirement A- The Contractor shall ride on top of each car and Verify the proper operation of the following devices: 1-Door locks 2-Limit switches 3-Exit switches 4-Emergency stop switches 5-Pit switches B? If an elevator fails the inspection/ testing one additional inspection must be included as part of the contract, at no additional cost to the Government. C- All elevator testing should be performed in accordance with the ASME Standards. D? All work should be completed by a QE (Qualified Elevator Inspector) accredited by the American Society of Mechanical Engineers. To receive full text of Scope, please contact Karen Sims on 215-861-1222 or e-mail Karen.Sims@gsa.gov. 06- The Contractor shall provide all of the appropriate completed inspection forms, supporting documents Elevator Performance Check (GSA 2480), Elevator Inspection Report form 376, 871, and the Certificate of Elevator Inspection (GSA 55). When the entire inspection for each building are completed GSA should receive all of the above documents 14 days after completion of the inspections. 08- The GSA Representative for this project is Jim Begley Telephone number (215) 861-3621. 09- Time for completion shall be 45 days after receipt of notice to proceed.
