Description: INSERT FULL TEXT OF PERFORMANCE-BASED WORK STATEMENT AS FOLLOWS: PERFORMANCE-BASED WORK STATEMENT DEEP CLEANING OF TOILETS AND SANITARY DISPOSAL AT RAF MOLESWORTH 1. DESCRIPTION OF SERVICES/GENERAL INFORMATION Contractor shall accomplish all cleaning tasks to meet the completeness and quality set forth in this Performance Work Statement and frequency requirements. 1.1. Services under this contract will usually be accomplished during the hours of 0730 through 1630 Monday through Friday, U.S. Holidays excluded. Any additional holidays declared by the U.S. Government shall likewise be considered official holidays for purposes of this provision. 1.2. The Contractor shall provide all personnel, supervision, equipment, materials, tools, transportation, and other items and services necessary to perform Deep Cleaning/Sanitary Disposal in the Joint Analysis Center (JAC) at RAF Molesworth. Contractor furnished items and services shall comply with commercial standards, Air Force Occupational Safety & Health Standards and base fire regulations. Upon request, contractor shall submit product literature on all supplies for review by the Facility Manager prior to use. Samples may be requested for review. 1.2.1. DISINFECTANT/DETERGENT. The disinfectant/detergent shall be currently Environmental Protection Agency (EPA) approved and registered as a pseudomonicidal, fungicidal, tuberculocidal, and viricidal at the recommended use dilution even in hard water of 400 PPM (CaC03). The disinfectant must be presented to the ICC for approval prior to use. All containers of cleaning chemicals and similar products shall be conspicuously marked (preferably with a factory label) to identify contents, and all other labels shall be removed or defaced. Materials bearing a Department of Transportation (DOT) red label (flammable) shall not be used. 1.2.2. CERTIFICATIONS. The contractor shall be responsible for obtaining all certifications, licenses, permits, and associated documentation to perform all services listed in the Performance Work Statement (PWS), before starting work. 1.3. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the Contracting Officer as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population. 1.4. Contractor personnel shall wear appropriate company name badges and uniforms at all times during performance of this contract. 1.4.1. Personnel shall wear personal protective equipment (PPE) as directed, when exposure to bodily fluids is anticipated. 1.5. Quality Control. The contractor shall develop and maintain a Quality Control Plan to ensure all services listed in this Performance Work Statement are performed in accordance with commercial standards. The contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective services 1.6. The Contractor shall be responsible for creating, maintaining, and disposing of only those government required records which are specifically cited in this performance work statement or required by the provisions of a mandatory directive listed in Section 4.1., Applicable Publications and Forms. If requested by the Government, the Contractor shall provide the original record, or a reproducible copy of any such records within five (5) working days of receipt of the request. 1.7. DEEP CLEANING. Deep Cleaning service includes the cleaning and care of the following items: Descaling of sanitary ware, paying special attention to overflows and waste outlets; dismantling, descaling, and thorough cleaning and reassembling of waste and traps; thorough cleaning of metal work (including stainless steel) using the appropriate metal polish and dry buffing; steam cleaning of all sanitary ware using a steam cleaner to remove latent bacteria, germs and remaining body fats. Tasks shall be performed in the rooms identified in Technical Exhibit1. Each toilet facility shall be deep cleaned every month. 1.8. SANITARY DISPOSAL. Disposal of sanitary facilities in women?s restrooms to be accomplished monthly. 1.9. The contractor shall establish a work schedule to include all required tasks. The contractor shall provide the QAP a copy of the work schedule at contract start date, and whenever changes occur 1.10. During normal duty hours fire and security protection issues should be reported to the QAP 1.11. Facility defects found shall be identified and reported, in writing, to the QAP by the end of the working shift. If the QAP is not on duty at that time, the report will be made to the Administrative Officer of the Day (AOD). 1.12. QUALITY ASSURANCE. The U.S. Government will evaluate the contractor?s performance as follows: a) AF Form 714 ? Customer Complaint Record. The Quality Assurance Personnel shall make final determination of the validity of customer complaint(s), in the case of disagreement between the Contractor and a customer. b) Periodic inspections of the Contractor?s Quality Control Plan (for example: contractor?s inspection schedule). 1.13. Re-performance Of Services. Should inspection results reveal that services were not performed at all or not performed in accordance with the stated standards listed in this Performance Work Statement, the contractor shall re-perform a task and/or all tasks rated ?Unsatisfactory? by the Quality Assurance personnel at no cost to the U.S. Government. 1.14. In conjunction with the Inspection of Services - Fixed Price clause, FAR 52.246-4 and Contract Terms and Conditions -- Commercial Items clause, FAR 52.212-4 the government reserves the right to make a monetary adjustment for non-conforming service and inadequate Quality Control. The Government shall utilize the following method for determining monetary adjustments: (a) Determine monthly line item cost per unit by using the bid schedule and or contractors pricing documents. (b) Determine the quantity of the service performed per month. (c) Determine the quantity of unacceptable service. (d) Divide the quantity of unacceptable service by quantity of service performed per month. (e) Multiply the percentage of unacceptable service by the line item cost per month for the monetary adjustment. 1.15. NORMAL HOURS OF OPERATION. The Contractor shall perform the services required under this contract during the following hours: Normal Hours: Monday through Friday 0730 to 1630 1.16. HOLIDAYS: The Contractor is not required to perform service on the following days: United States (U.S.) Holidays New Year?s Day 1 January Martin Luther King?s Birthday 3rd Mon in January President?s Day 3rd Mon in February Memorial Day Last Mon in May Independence Day 4th July Labor Day 1st Mon in September Columbus Day 2nd Mon in October Veterans Day 11th November Thanksgiving Day 4th Thurs in November Christmas Day 25th December When a holiday falls on a Saturday, the preceding day is also a holiday. When a holiday falls on a Sunday, the following day is also a holiday. Periodic services not performed due to U.S. holidays, will be performed on the following duty day. 1.17. Employees. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106. The Contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours or deployment. The abrupt absence of these personnel shall not constitute an excuse for nonperformance under this contract. 1.18. Performance Of Services During Crisis Declared By The National Command Authority Or Overseas Combatant Commander. Contractor access to the installation may be denied during a national emergency or crisis situation called by national command authorities, or overseas combatant commanders. Such circumstances are deemed beyond the government?s control and access to the installation may be limited in such circumstances on the basis of public safety and installation security. Should limited access be necessary, the JAC will inform the contractor in writing and identify the date when services may resume, if possible. The contractor will not be held liable for lost time during these situations. 1.19. Special Qualifications. No special qualifications are identified beyond those required by the government of the United Kingdom. 1.20. Security Requirements. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by all security regulations of the installation. When any employee no longer works for the contractor at the installation, the contract manager shall return any government furnished identification card to quality assurance personnel or Security Forces. Contractors shall expect delays entering the base and entry and exit of restricted areas. If, for any reason, Security Forces or any military member detains a contractor or contract employee, the contractor is to comply with their request and is not to become combative or argumentative. Contractors will be offered the opportunity to undergo Antiterrorism Level I training prior to work start. The training will not exceed 30 minutes and will be conducted at RAF Alconbury by the Installation Antiterrorism Officer. 1.20.1 Escort Requirements. Contract personnel will require escorting in all JAC facilities as identified in Technical Exhibit 2. In those buildings requiring escort, the contract personnel will call the telephone number clearly identified on the outside of the door to gain admittance. 2. SERVICES SUMMARY: PERFORMANCE OBJECTIVE PWS REFERENCE PERFORMANCE THRESHOLD All services should be in compliance with Air Force Occupational Safety and Health Standards and other base regulations. Para. 1.2. 100% All disinfectant/detergent shall be Environmental Protection Agency (EPA) approved and appropriately registered. All cleaning materials/containers should be properly marked. Materials with Dept. of Transportation (with red label/flammable) shall not be used. Para. 1.2.2. 100% The contractor is responsible for obtaining all certifications and licenses, permits and associated documentation in performance of all listed services in the PWS. Para. 1.2.4 100% Contractor shall clean and disinfect restrooms and perform sanitary disposal tasks Para. 1.7 & 1.8 100% As specified on the attached technical exhibits 3. GOVERNMENT FURNISHED PROPERTY/SUPPLIES 3.1 Not Applicable 4. APPENDICES. 4.1. APPLICABLE PUBLICATIONS AND FORMS Publications and forms applicable to this Performance Work Statement are listed below. The publications and forms are coded as mandatory or advisory. The Contractor is obligated to follow those publications and to use those forms coded as mandatory to the extent specified in the PWS. The Contractor shall be guided by those publications or use those forms coded advisory to the extent necessary to accomplish requirements in this PWS. The publications and forms may be accessed electronically at this address: http://afpubs.hq.af.mil. Supplements or amendments to listed publications from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those changes in publications, which result in a decrease or no change in the contract price and notify the Contracting Officer, in writing, of such change. Should a decrease in contract price result; the Contractor shall provide a proposal for a reduction in the contract price to the Contracting Officer. Prior to implementing any change that will result in an increase in contract price, the Contractor shall submit to the Contracting Officer a price proposal within thirty (30) days of receipt of the change by the Contractor. The Contracting Officer and the Contractor shall negotiate the change into the contract under the provisions of the contract clause entitled "Changes". Failure of the Contractor to submit a price proposal within thirty (30) days from receipt of the change shall entitle the Government to performance in accordance with such changes at no increase in contract price.
