- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 and 13.1, as supplemented with additional information included in this notice. This solicitation, 292-06-P(HQ)-0304, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) Number 05-2006. The North American Industry Classification System is 811219. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The intent of this solicitation is to award one firm-fixed price (FFP) contract for Sterilizer Maintenance & Repair. The contract will consist of the base year and four, 12-month option periods. Award will be made without discussion to the offeror proposing the best value to the government, based on past performance, experience of technical personnel, timely response and cost/price. Base Year April 12, 2006 through April 11, 2007 ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Sterilizer Equipment Preventive Maintenance (FFP) 1 YR Non-personal service: Contractor shall provide all labor, parts, tools, equipment, and transportation necessary to maintain sterilizer equipment in accordance with the Performance Work Statement. FOB: Destination Option Year 1 April 12, 2007 through April 10, 2008 0002 Sterilizer Equipment Preventive Maintenance (FFP) 1 YR Non-personal service: Contractor shall provide all labor, parts, tools, equipment, and transportation necessary to maintain sterilizer equipment in accordance with the Performance Work Statement. FOB: Destination Option Year 2 April 11, 2008 through April 10, 2009 0003 Sterilizer Equipment Preventive Maintenance (FFP) 1 YR Non-personal service: Contractor shall provide all labor, parts, tools, equipment, and transportation necessary to maintain sterilizer equipment in accordance with the Performance Work Statement. FOB: Destination Option Year 3 April 11, 2009 through April 10, 2010 0004 Sterilizer Equipment Preventive Maintenance (FFP) 1 YR Non-personal service: Contractor shall provide all labor, parts, tools, equipment, and transportation necessary to maintain sterilizer equipment in accordance with the Performance Work Statement. FOB: Destination Option Year 4 April 11, 2010 through April 10, 2011 0005 Sterilizer Equipment Preventive Maintenance 1 YR Non-personal service: Contractor shall provide all labor, parts, tools, equipment, and transportation necessary to maintain sterilizer equipment in accordance with the Performance Work Statement. FOB: Destination The Contractor shall provide all personnel, labor, supervision, materials, parts, tools, equipment, expertise, and other necessary services to perform preventive maintenance and repair to the equipment listed in the statement of work. All requirements are located at the National Institute of Environmental Health Sciences, Research Triangle Park, N.C. All work shall be performed during regular Government working hours of 8:30 a.m. to 5:00 p.m., exclusive of Government recognized holidays. Any variation of work time shall be requested to and approved by the Project Officer in advance. Before work is started, the Contractor shall confer with the Project Officer to arrange a work schedule satisfactory to both the Contractor and the Government. The Contractor shall comply with all Institute regulations incorporated herein governing the occupied premises and shall perform the Contract in such a manner as not to interrupt or interfere with the conduct of Government business. The Contractor shall adhere to all parking rules and regulations governing the use of the premises. Definitions: Preventive maintenance ? scheduled checks and services (including, but not limited to, adjustments, cleaning, calibration, inspection, and equipment servicing) of installed equipment at intervals recommended by the manufacturer or standards within the industry to ensure proper and efficient operation with minimum breakdowns, deterioration, or other deficiencies. Preventive maintenance shall also include repairs or other actions taken to correct deficiencies discovered during preventive maintenance inspections. Repairs ? The Contractor shall repair all equipment listed in the description and specifications, so as to maintain its designed operating capability. Repairs may be accomplished by replacement or restoration of component parts or materials that have deteriorated, worn out, or failed and are unable to perform to the manufacturer?s specifications. The Contractor shall provide only trained personnel with 5 years experience in the repair, maintenance, and installation of the equipment types listed herein. The Contractor shall require each employee to personally sign in or arrival at NIEHS and sign out when departing NIEHS. The location for signing in and out is in Building 102 and Rall Building, the first floor of Module B, South Campus. Parking for the Contractor?s employees shall be in accordance with the NIEHS policies, unless directed otherwise by the Project Officer. Each Contractor employee working at the Institute shall wear a name badge, easily readable, prominently displayed, which shall include the person?s name and the Contractor?s name. The Contractor shall provide the Project Officer within two (2) days of Contract award with a typewritten list of the personnel and their telephone numbers or other means of contacting the appropriate persons for repair services at any time during the day. This information shall be updated as necessary by the Contractor so as to be current and accurate at all times. Specific Performance Requirements ? The Contractor shall provide the following services: Preventive Maintenance ? The Contractor shall provide preventive maintenance (PM), which is currently scheduled through the NIEHS Preventive Maintenance System, on a bi-monthly basis. The preventive type maintenance (inspection, lubrication, adjustment, etc.) shall be accomplished on the equipment in accordance with the Equipment Checks (EQ) from the PM System and the manufacturer?s design. PM Checksheets ? The Contracts Management Section (CMS) has prepared PM work orders for the equipment listed in the description and specification. The Contractor?s preventive maintenance effort shall include, as a minimum, accomplishing all the checks specified on the work orders for the specific equipment being serviced. Copies of these work orders will be provided to the Contractor before the month scheduled in order for the Contractor to perform the work. The Contractor shall accomplish the preventive maintenance and submit the completed work orders to the Project Officer by the last day of the month for which the checks are scheduled. The Contractor shall conform to the Equipment Checks and use these instructions/procedures in performing the preventive maintenance tasks. Schedule and Advance Notice of Preventive Maintenance The Contractor shall provide the Project Officer with a schedule of the intended date for accomplishing the preventive maintenance for all equipment identified. The Contractor shall notify the equipment users (the laboratory staff who use the particular sterilizer) not less than 48 hours in advance of performing the scheduled preventive maintenance. All work shall be scheduled to be performed at a time convenient to the user, unless directed otherwise by the Project Officer. Repair Service The Contractor shall perform all repairs as required to restore equipment to optimum operating performance. Notice for repairs shall be placed by the Project Officer or designee. The Contractor shall be on site and initiate repairs to the equipment listed herein within 24 hours (one work day) after a call has been placed by the Project Officer or designee. The Contractor shall restore equipment to service as soon as possible. For equipment expected to be out of service for extended periods of time, greater than five work days, the Contractor shall provide the Project Officer with a written report of the repair problems, the steps that the Contractor has taken to remedy them, and the expected date that the equipment shall be returned to service. A written update to this report shall be made as often as the Project Officer requests the report, but not less than every twenty-one (21) days thereafter for so long as the equipment is out of service. Repair requirements shall not be accumulated or backlogged. Each repair accomplished shall be a complete repair. Temporary repairs will not be allowed unless specifically approved by the Project Officer in advance. Clear justification for temporary repairs must be provided in writing and accepted by the Project Officer in order to obtain approval. Replacement Parts and Services All necessary repair parts and materials for maintenance and repair of the equipment listed herein shall be furnished at no additional cost to the Government by the Contractor. The Contractor shall provide and install manufacturer?s approved parts for repairs. Repair parts used by the Contractor shall be warranted for one year. Equipment shall be maintained by the Contractor to the manufacturer?s specifications and technical updates, unless authorized otherwise in writing by the Project Officer. All parts furnished and workmanship performed by the Contractor shall be of a high quality and acceptable to the Government. The Contractor shall immediately rectify unacceptable work at no cost to the Government. Service Reports The Contractor shall provide a service report to the Project Officer upon completion of all repair work. The report shall identify the equipment, the printed name of the technician making the repair, the start date and time of the repair, the completion date and time of repair, the signature of the equipment user (the NIEHS laboratory staff who uses the sterilizer) verifying that the equipment is repairs and operational. A copy of the signed service report shall be left with the NIEHS official (user) responsible for the equipment under repair. The Contractor shall present the original of each Service Report to the Project Officer immediately upon completion of the service. General Work Performance: The Contractor shall clean the area of work of any contract effort. This clean up shall include disposal of all empty containers, packing materials and debris. All components and hardware shall be removed or otherwise disposed of. All spills shall be removed and the surfaces cleaned. The Contractor shall not create or allow to exist any safety or fire hazard. All non-burnable material must be removed from the Institute by the Contractor. The Project Officer may direct disposal of the metal parts and components or other items as required. The Contractor shall comply with all hazardous waste disposal requirements. The areas shall be left in clean and orderly condition, free of any foreign material generated by the preventive maintenance and repair activities upon completion of each service. The Sterilizer Equipment includes two Forma Scientific Model 6385 Hot Air Sterilizer 33x74x74; one -Amsco Lab ? TCC 24x36x36; one -Amsco Century Model SV-116 20x20x38; one -Amsco Lab G-120 Type Century,20x20x38; one -Amsco Scientific S/G 120 and one -Amsco Lab G-120 Type Century 20x20x38 20? Vac. A complete quotation package will include: 1) Price for each item number. 2) Signature of the offeror on the page that lists the price. 3) A detailed narrative addressing how your company?s services meet the technical capability criteria as listed in this solicitation should be of sufficient detail to determine adequacy. 4) Past performance of contracts similar to this requirement completed in the past three years. Provide Contracting Activity Name, contract number, Contracting Officer and telephone number, contract value, brief project description; 5) Completed copy of the provisions at FAR 52.212-3, Offeror?s Representations and Certifications ? Commercial Items or confirmation that this has been completed online. If an offeror?s quote does not identify all items listed above, the quote may be considered incomplete for evaluation purposes, and no further consideration will be given to the offeror?s quote, thereby making an offeror ineligible for award. Each offer should contain the offeror?s best terms from a price and technical standpoint. Award will be made to the responsible offeror whose offer conforms to the solicitation and represents the best value to the Government. Before award is made, offeror must be registered in the CCR.
