Description
The VA Medical Center located in Alexandria, Louisiana has a need for an Urologist for the period 17 Apr 06 ? 30 Sep 06. Services are required five (5) days a week Monday through Friday 8:00 AM ? 4:30 PM excluding Federal holidays, with a 30-minute unpaid lunch period. Night and weekend call will be shared with other Urologists. All services associated with a Board Certified/Board Eligible Urologist is required for general clinic, cystoscopies and prostate biopsies. Contract services assist the staff Urologist to reduce waiting time for veteran referrals. The North American Industry Classification System Code is 621111 with $5.0 million annual sales limitation for small business. VA intends to award a firm-fixed price, non-personal services contract for Health Care Resources in accordance with Public Law 104-262 under the authority of Title 38 U.S.C. 8153. Required services include outpatient/inpatient clinic services. The Urologist shall be skilled, knowledgeable, and experienced in all facets of adult Genito-Urinary care as established by industry and VA standards. He/She shall be in good standing with the profession with knowledge and abilities current to equipment, medication and techniques. The Board Certified Urologist shall provide comprehensive health services relating to all phases of Urological complaints and diseases, routine and malignant. The physician shall perform ambulatory surgical procedures to include, but not limited to cystoscopies, stent placement, urethral dilatations, and transrectal needle biopsies of prostate. There is some night, weekend, and holiday call. Services shall be provided at VA Medical Center, 2495 Shreveport Highway, Building 7, Urology Clinic, Alexandria, LA. Routine workload is approximately 35 patients per day. Documentation shall be performed in accordance with all VAMC established principles, ethics, standards, rules and regulations as set forth in VHA Directives, Medical Staff Bylaws, Medical Center Memorandums, AMA, and JCAHO standards. Qualifications: The Contract Urologist shall have an active, current, unchallenged and unrestricted license in a state, territory, or commonwealth of the United States or District of Columbia and shall be board certified/board eligible. He/She shall speak, understand, read and write the English language fluently; be computer literate and familiar with VA's electronic Clinical Patient Record System (CPRS) to input and access documentation. The contractor shall maintain malpractice professional liability insurance in an amount not less than $1,000,000 per occurrence for the entire contract period. Solicitation RFQ 502-18-06 will be available on or about April 3, 2006.
