- Summation iBlaze SSE DITSCAP Compliant Software This is a combined synopsis/solicitation for commercial items prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is H94002-06-Q0041, as changed by amendment 0001, and will be issued as a Request for Quotation (RFQ). Amendment 0001 changed the format of the RFQ from an SF1449 to an SF18, extended the due date to April 10, 2006 and changed the Statement of Work to verbiage reflected below in the description of services. The solicitation is being issued as a single source with a response due date of April 10, 2006, not later than 12:00 pm MST. FOB Destination shall be Aurora, CO, 80011 and the contract action will result in a Firm Fixed Price contract. The Contractor shall be registered in Central Contractor Registration (www.ccr.gov) before award can be made. Send quotations to Mr. Daniel H. Poturalski, Office of the Assistant Secretary of Defense Health Affairs, TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, Colorado 80011-9066, or by facsimile to 303-676-3987. Facsimile quotes should be followed by signed, hardcopy forwarded to the address stated above. The quotation submitted by the contractor against this requirement shall clearly state the following: Request For Quotation Number; the time specified for receipt of offers; Name, address, telephone number of the Offeror?s point of contact; Terms of warranty; price; and discount terms and acknowledgement of all quotation amendments (if applicable). TRICARE requires the following: Description of Services - Summation iBlaze SSE DITSCAP Compliant Software I. GENERAL The Contractor will provide deliverables as described in paragraph III. below, to the TRICARE Management Activity (TMA) Office of General Counsel (OGC) and Information Management, Technology, and Reengineering Directorate (IMT & R), (hereafter referred to as TMA-OGC and TMA-IMT&R respectively). II. SCHEDULE Deliverables will be required in accordance with the following schedule: 0001 Installation of software April 10, 2006 0002 40 User Summation Software May 12, 2006 0003 Maintenance Date of Software Purchase through 12 months. 0004 Training May 15, 2006 0005 SQL Migration Kit May 12, 2006 III. DELIVERABLES 1 Installation 1.1 The Contractor shall provide installation of Summation iBlaze SSE DITSCAP compliant software at TMA?s Aurora, CO location. 1.2. The required end-results of the onsite SSE installation service are as follows: 1.2.1. Installing 40 User Summation iBlaze SSE DITSCAP compliant software. 1.2.2. Creating and configuring the initial SQL databases, including the Summation Master (Universal Licensing Manager ? ULM) database. 1.2.3. Setting up basic user groups, primary administrator accounts, and a standard user for testing purposes. 1.2.4. Installing the fuzzy vocabulary server. 1.2.5. Installing network client software on the administrator's desktop for testing and administrative use. 1.2.6. Installing and configuring the P. Franc v. K. Morris demonstration case for testing and administrative use. 1.2.7. Shall include any/all costs associated with travel, lodging, and food. 2. 40 User Summation Software 2.1. Software is to be Enterprise 2.0 Network and License. 2.2. Software is to be DITSCAP Compliant. 2.2.1. Requires passing the MHS IA Security Assessment. 2.2.1.1.Signature of a DITSCAP Authorized Individual. Offeror shall perform, as necessary, to pass the MHS IA Security Assessment within a mutually acceptable period of time. The dates identified in Schedule II are target dates, contingent to mutual acceptance. In the event that certification is unobtainable within the agreed period of time, Contract Line Items 0002 through 0005 will be terminated at no cost, for the Convenience of the Government. 2.3. Shall include costs associated with shipment of the software 3. Maintenance 3.1. Maintenance shall commence the date that the software is purchased and continue through a period of twelve months. 4. Onsite Administrative Training: 4.1. The Contractor shall provide onsite Summation iBlaze SSE Administrative Training service for 2 TMA-IMT&R employees stationed at TMA?s Aurora, CO location. 4.1.1. Shall include any/all costs associated with travel, lodging, and food. 4.1.2. Shall include all educational materials as necessary. 4.2 The required end-results of the onsite SSE Administrative Training service are as follows: 4.2.1. The training shall enable the employees to fully function as system administrators of the software, and include training which will allow for proficiency in all available administrative tasks, functions and features, including the Administrator console and SQL Administration feature of Summation SSE. 4.2.2. Shall require signed acceptance by the Contracting Officer Technical Representative that the training objectives have been satisfied. 5. SQL Migration Kit TRICARE is the name of the Department of Defense?s managed health care program for active duty military, active duty service families, retirees and their families, and other beneficiaries. The offeror must comply with the following commercial item terms and conditions: FAR 52.212-1, Instructions to Offerors Commercial Items; FAR 52.212-3 Offeror Representations and Certifications Commercial Items, MUST be completed and submitted by the offeror with the response to the RFQ; FAR 52.212-4 Contract Terms and Conditions Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items ? Deviation.
