Description
Solicitation for the possible replacement of Blood Collection Devices for TRICARE Northwest Region. These products include but are not limited to a complete product line of blood collection products. Those companies interested in a possible Regiona l Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Northwest Region must respond to the following questions: Clinical Criteria: Safety Butterfly Devices 1. Safety mechanism is activated with one hand. 2. Needle is sharp and free of burrs. 3. Bevel is smooth. 4. Surface of wings that touch the patients skin is smooth and free of rough edges. 5. Once safety device is activated, no part of the needlepoint is accessible. 6. Winged component is made of inert material, which will not cause skin irritation. 7. Winged device is adaptable for ambidextrous use. 8. Winged device is easily compatible with associated (same manufacturer) transfer and collection components. 9. Device available in the various needle sizes (19, 21, 23, and 25 gauge). 10. Compatible with and without multi-luer adapters. 11. Compatible with blood tube holders. 12. Compatible with safety transfer devices. Safety Blood Transfer Device 1. This device is compatible with products currently in use. 2. The device makes for safer transfer/acquisition of blood. 3. Minimal changes in technique and use are required. 4. The safety device does not interfere with the intended use. 5. Use of the device did not create any extra risk of sprays, blood leakage, and/or drips. 6. The fittings (luer loc/slip tip or other) are snug, with no leakage. Blood Tube Criteria 1. Tube made from material that allows undistorted view of contents. 2. Tubes are capable of retaining vacuum for stated shelf-life. 3. All materials used in construction of the container and any lubricants, coatings or additives do not adversely influence the results obtained. 4. Tube provides closed system for needle puncture and reseal. 5. Closure is designed so it can be removed by gripping with fingers or by mechanical extraction. 6. Tube provides surface for patient identification information. 7. Tubes come in variety of sizes. 8. Full line of tubes is available. Vacutainer and Vacutainer Needle Criteria 1. One handed safety device. 2. Inner vacuum tube needle (rubber sleeved needle) does not present a danger of exposure. 3. A clear change (audible or visible) occurs when item is activated. 4. Device available in 21 and 22 Gauge. 5. Needles packaged individually. 6. Needle is compatible with vacutainer holder. Safety Lancet Criteria 1. The needle is not exposed before or after use thus minimizing the risk of needle-stick injury. 2. Incorporates the lancing device and the needle into a complete disposable unit that is used only once. 3. No arming of lancet is required. 4. Lancets are color-coded for easy recognition. 5. Once used, lancet is rendered inoperative providing added safety. Blood Collection Technical/Company Criteria 1. Are you a manufacturer or distributor of blood collection products? Provide evidence that you manufacture/distribute a full line of blood collection products. 2. Is your product line manufactured using approved industry standards, i.e., comply with OSHA, FDA, ACAP standards and guidelines? 3. Does your company provide compliance programs to support infection prevention efforts? 4. Does your company have a distribution agreement with Cardinal Healthcare? 5. Does your company currently possess, or in the process of obtaining, a Distribution and Pricing Agreement (DAPA)? 6. Does your company have the ability to service to the entire TRICARE Northwest Region (WA and Alaska)? Please provide your companys distribution network. 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will pro duct in-service and education be available upon request after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Northwest Region. 10. Does your company provide volume discounts? 11. Provide a history of back orders and/or recalls for this product group with dates, duration, cause, and how information was communicated to the customer, interim measures and final resolution. 12. Does your company have a 1-800 # for customer questions? 13.
