Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation WILL NOT be issued. Solicitation Number V3047-0638 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-08. The NAICS Code is 339112 and the small Business Size is 500 employees. This action is 100% Set Aside for Small Business. The Veterans Affairs Special Services (VASS) Contracting Activity has a requirement for a Visual Acuity System with accessories; the Manufacturer is M&S Technologies. The quotation shall include the following: Line Item 0001. 17 EA. – Smart System II PC-Plus. Includes VAFCS Software, Central Processing Unit, Programmable Keypad Controller, 50’ Extension Cable, NEC 17” flat screen LCD monitor with 400:1 contrast ratio, dual speakers, Snellen & Sloan character set, LEA Symbols & Numbers, Allen Pictures, Tumbling E’s Cycling Letters, Crowing\Interaction Bars, HOTV, Red\Green, Optotype Pointer, Worth 4 Dot, Landalt C’s, Snellent\Logmar switch control, Adult Fixation Dot, 3 Lines Same Size, Vertical Line, Fixation System Module, Includes 3-part Foot Controller, Animated Targets, Fixation Software Module with 3 Video Source Modules, Video Select and Assign Function, Quick Copy Video Function, Multi-Media Monitor Base, JPG Image Select and Display Feature; also includes Wireless Infrared Remote Control and Receiver, P/N SS2K100F/FW/LCD “Or Equal”, Unit Price _______, Total Price________; Line Item 0002. 17 EA. – Digital Animated Fixation Targets, “Or Equal”, Unit Price _______, Total Price_________; Line Item 0003. 68 EA. – Video Cartoons (4 per system), “Or Equal”; Unit Price _______, Total Price_________; Line Item 0004. 17 EA, Wall Mount Assembly for CPU and LCD. Allows for both units to mount flush to the wall. The LCD covers the CPU for an aesthetically appealing installing with adjustable Tilt, P/N200LCDCPU “Or Equal”, Unit Price _______, Total Price_________; Line Item 0004. 1 EA. – Installation of Systems, Unit Price _______, Total Price_________. FOB Destination to Lackland AFB, TX 78236. Delivery within 45 calendar days after receipt of award, to 59TH MDW/MSLE 2200 Bergquist Dr., Suite 1, Lackland AFB, TX 78236-5300. “ALTERNATIVE ITEM BIDS MUST MEET OR EXCEED THE ITEM REQUESTED AND MUST INCLUDE SPECIFICATIONS, MANUFACTUR’S PART NO. AND DESCRIPTIVE LITERATURE SUFFICIENT FOR REVIEW AND COMPARISON”. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, which represents lowest evaluated cost or price, technically acceptable bid. Technically acceptability will be evaluated based on the function and operation of the item in accordance to Government's specifications. Offerors shall submit descriptive literature, product samples, technical features, and warranty provisions. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer as applicable. The following additional clauses apply: CCR Clause: Attention: You must be registered in CCR to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the Offeror shall provide its DUNS or, if applicable, it’s DUNS+4 number with its offer, which will be used by the contracting officer to verify that the Offeror is registered in the CCR database. (C) Lack of registration in the CCR database will make an Offeror ineligible for award. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. VAAR 852.219-70, Veteran-owned Small Business; 852.270-4, Commercial Advertising. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, company name, and point of contact, address, phone number, fax number, tax identification number, DUN number, and size status (i.e., small business, disadvantaged business or woman owned business). The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
