- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. ***************************************************************** Purchase Request Number: F3N7256039A100 - Request for Quotation ? This document incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-07. ***************************************************************** Line Item 0001: 1-year lease contract for one each panel van (i.e. Vario or Sprinter) Required vehicle modifications: Metal partition between driver and truck bed: No windows in bed or partition, hasp lock installed on truck and side entrance. Required Service: Insurance-fully covered. No deductible. Line Item 0002: 1 year lease contract for one each station wagon Required Service: Insurance-fully covered. No deductible. *** Period Of Performance: 01 May 06 ? 31 April 07 Quote should also include one option year. *** STATEMENT OF WORK 1. Vendor must provide telephone numbers for seven day a week, 24-hour a day notification. a. Other requirements: 1) Vendor must maintain full coverage insurance with no deductible and proof thereof on each vehicle. 2) Vendor must deliver vehicle with a full tank of fuel. 3) Vehicles must be delivered cleaned and serviced. 4) Vehicle must be delivered with all applicable documentation to include registration, insurance, inspection, and vehicle manuals. 5) Scheduled maintenance of vehicle is the vendor?s responsibility to include but not limited too: All general maintenance (i.e. oil changes, tire rotation and replacement, break fluid, etc.) If a vehicle requires minor or major maintenance to be performed, a replacement vehicle must be provided immediately. 6) Vendor will notify customer no later than one-week prior of any scheduled maintenance that needs to be performed on any vehicle. 7) Temporary replacement vehicle ? will be provided in the event of break-down or accident within 24 hours 8) Temporary and/or replacement vehicle must meet contract specifications and modifications standards 2. Required panel van modifications: Van must be of cargo capacity type with separate lockable enclosed cargo area separated physically from driver/passenger area, no windows in truck bed or partition, hasp lock installed on truck rear doors and side door entrances (if present), and hasps must be able to accept lock with 1/2 inch shank 3. Vendor shall provide the requested vehicle types, with-in 24 hours of the call 4. A base safety representative has the authority under this contract to conduct random and unannounced safety checks of all Vendor provided vehicles. 5. Only a Contracting Officer is authorized to make changes to the terms and conditions of this contract. 6. Vehicles are subject to rejection if one or more of the following areas are found unacceptable. If a vehicle is rejected, the vendor must provide a replacement within three hours: a. Windows: Shall be free of all cracks, holes, etc? b. Tires: Shall have proper tread, free of all cracks, splits, and cuts, etc. c. Lights: Shall be operational, to include brake lights, signal indicator lights, headlights, parking lights and fog lights if installed. d. Horn: Shall be operational e. Cleanliness: Interior and exterior of vehicle shall be free of all dirt, trash and debris. f. Brakes: Shall be in good working order (i.e. brake shoes/pads, etc.). g. HVAC: Shall have operational heating and air vents along with a window defogger. h. Air-conditioning: Shall have operational air-conditioning. i. Exhaust System: Shall be in good working order. (I.e. free of leaks, holes, etc). j. Doors: All doors will shut and lock properly without the aid of any foreign objects (i.e. wire, etc). k. Seatbelts: Must be installed in ?all? passenger-seating positions. l. Miscellaneous: 1) Shall have all safety items required by European and Belgian Law 2) Shall have an operational jack to install the spare tire 3) Shall have a spare tire, reflective vest, extinguisher, and emergency/safety kit 4) A set of serviceable chains for snow/ice must be supplied with each vehicle during winter months. 7. Fixed monthly price for all vehicles - not based on mileage 8.
