- This is combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announce constitutes the only solicitation; quotes are bein g requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-8050. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect throu gh Federal Acquisition Circular 2005-07. The North American Industry Classification System Code (NAICS) for this procurement is 339113. The Martin Army Community Hospital, Fort Benning Georgia 31905-6006, has a requirement for 15 EZLOC SCREWS OF VARIOUS SIZES, 6 WASHERLOC OF VARIOUS SIZES, 12 CORING TREPHINE OF VARIOUS SIZES, 38 CANCELLOUS SCREWS OF VARIOUS SIZES, AND 50 CORTICAL SCREWS OF VARIOUS SIZE. Please contact Mr. Raymond E. Hulse to inquire about specific sizes. Please include shipping cost and date of shipment. The Government anticipates awarding a single award for this requirement. The items shall be delivered to the Martin Army Community Hospital, Instl Med Sup Acct, ATTN: Cora Redding, Bldg 9222, Santa Fe Road, Fort Benning, Georgia 31905 as soon as possible after receipt of order.
