Description

The VISN 10 Healthcare System is seeking a contractor to furnish all storage, delivery, set-up, in-house instructional training, labor, transportation, and equipment required to provide durable medical equipment (DME) and Home Oxygen. Install and/or remove portable ramping, porch or trunk lifts; and store and assemble/deliver power wheelchairs and scooters. Along with deliver of equipment to the veteran?s residence, it shall be properly assembled and tested. The Government-owned items and equipment are for beneficiaries of the VISN 10 geographic area. The geographic area is comprised of five (5) VISN sites: (1) Louis Stokes Cleveland VAMC, Wade Park Unit, 10701 East Blvd., Cleveland, OH 44106, which also includes the Brecksville Unit at 10000 Brecksville Rd, Brecksville, OH 44141; twelve (12) clinics (see paragraph 3.1 for locations of clinics); (2) VAMC Cincinnati, 3200 Vine St., Cincinnati, Ohio 45200; (3) Chalmers P. Wylie Veterans Clinic, 542 Taylor Rd., Columbus, Ohio 43203; (4) Dayton VAMC, 4100 West Third Street, Dayton, Ohio 45428; and (5) Chillicothe VAMC, 17273 State Route 104, Chillicothe, Ohio 15601. The Government intends to award a Fixed-Price Requirements type contract to include a base year plus four, one-year optional renewal periods. Multiple Awards are anticipated for this requirement. Unless specified otherwise, all requirements, terms, and conditions within this solicitation are applicable for all five (5) VISN sites Chillicothe, Cincinnati, Cleveland, Columbus, and Dayton, Ohio. The solicitation will be available on or about May 15, 2006 on the FedBizOpps website at: FBO http://www.fbo.gov. This solicitation is a Total 100% Set-Aside for Small Business Concerns. The North American Industry Code System (NAICS) is 621610.
