- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W911YN-06-T-0002 must be referenced in your response. This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001 -24, Effective 18 Jun 2004 & Class Deviation 2004-00001 and Defense Federal Acquisition Regulation Supplement DCN 20040608 Edition. The Government intends to establish a Blanket Purchase Agreement (BPA) for this item which will be effective for the period of o n or about 1 May 2006 through 31 May 2011. This synopsis/solicitation is for small business set-aside. The North American Industry Classification System code for this synopsis/solicitation is 312113 and the associated small business size standard is 500 em ployees.Item Name: Ice, crushed, packaged in 20lb bags. Ice will be palletized at least 100 bags per pallet. Vendor must be able to get pallets to rear of truck and Government personnel will offload with forklift. Empty pallets will be picked up by vendor on next delivery. Under normal circumstances orders will be at least six (6) pallets, however, there may be orders place for more than or less than six (6) pallets. Deliveries shall be FOB Destination to the Camp Blanding Joint Training Center, Ration Dist ribution Warehouse at 5629 SR 16 West, Bldg #5720, Starke, Florida. Normal delivery hours are from 8:00 a.m. and 3:30 p.m., Tuesday through Friday. However, during training periods, emergencys or other unforeseen circumstances, the vendor must be capable of responding on short notice and be able to make deliveries on Saturday, Sunday or Monday if required. If not able to respond on short notice vendor must provide minimum delivery period. When place orders all efforts will be made to give vendor 24 hour n otice. Vendors shall propose pricing to show any quantity discounts up the maximum estimated quantity. All deliveries completed under the BPA shall be accompanied by sales slips that shall contain the following minimum information: Name of supplier, BPA number, date of service request, call number, and itemized list of services furnished, quantity , unit price, and extension of each service, date service was provided. The government estimates, but does not guarantee, that the quantity of ice to be purchased on the BPA is 25,000 bags per year. The Government is obligated only to the extent of authori zed purchases actually made under the BPA. Individual purchases under this BPA are limited to $10,000.00 or less. A list of individuals authorized to purchase under this BPA, identified either by title of position or by name of individual, organizational c omponent, and the dollar limitation per purchase for each position title or individual shall be furnished to the vendor by the contracting officer. Proposals will be evaluated on the following factors: cost per bag, lead time from order placed to delivery of ice. Failure to maintain compliance of said regulation will be grounds for termination of agreement. Vendor shall include with their proposal a copy of their latest and current Sanitation Audit Report.
