Description

West Palm Beach VA Medical Center has a requirement for a sole source contract for adult day healthcare services. The contractor is Mae Volen Senior Center, Inc., sites located at Delray Beach and Boca Raton, Florida. Contractor will furnish to the beneficiaries for whom such care is specifically authorized by the WPB VA. It is understood that the type of patient to be cared for under this contract will receive these services as a substitute for institutionalization. The purposes of providing ADHC are to enable functionally imparied individuals to reside in supportive home environments rather than in nursing home care facilities; to facilitate the return of older veterans to their home and coordinate long term care there by reducing admission to hospitals and nursing homes; to maintain the older veteran at the highest level of functioning possible; to improve the quality of life for the participants among will include: medical services, nursing social services, assistance with activities of daily living, activity therapy, case management, nutrition service including meals and transportation. If there are no responses received by the closing date, the contract will be awarded to the above noted contractor. All requests must be submitted in writing.
