GLENN, JR./ (478) 926-6358 or fax at (478) 926-7549 e-mail at John.Glenn@robins.af.mil SOLE SOURCE ACQUISITION: TERADYNE, INC (TERADYNE ASSEMBLY TEST DIVISION) 600 RIVER PARK DR, NORTH READING PA 01864 DESCRIPTION OF REQUIREMENT: The Contractor shall provide all the labor, material, supplies, and transportation necessary to perform maintenamce and repair for two (2) Spectrum S9100 test stations located at Robins AFB GA in strict accordance with the incorporated Statement of Work. PERIOD OF PERFORMANCE: Basic year, with four (4) option years FOB: Destination , 52.247-34 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. The following clauses are applicable. 52.252-2 -- Clauses Incorporated by Reference (Feb 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. (Jun 1997) PERFORMANCE WORK STATEMENT (PWS) MAINTENANCE & REPAIR ON TWO (2) TERADYNE SPECTRUM S9100 Test Stations ROBINS AFB, GEORGIA 27 Mar 2006 1.0 Description of Services This Performance Work Statement PWS) describe the hardware and software services for the support of the two Teradyne Spectrum 9100 series test systems for the 402d Electronics Maintenance Group (EMXG) at Robins AFB, GA. The equipment requiring the coverage are: C-17 Automatic Test Equipment (CATE) serial number 0352551 and Depot C-17 Automatic Test Equipment (DCATE) serial number 0415551. These two systems are utilized in the repair process of Avionics Line Replaceable Units (LRU) and Shop Replaceable Units (SRU) for the C-17 Aircrafts. All Equipment is located in building 645. The contractor shall provide all equipment, materials, parts, tools, labor, and transportation (including travel and per diem) for the hardware & software support of equipment listed in section 5.0 Appendices. 1.1 All tasks delineated herein are for the purpose of supporting the operation of the listed Teradyne Test Stations. The operation of these stations includes hardware assistance, preventive maintenance, component replacement, and repair. It will also include software maintenance of the testers. 1.2. Hardware 1.2.1. Service and repair of the listed Teradyne Test Stations in Section 5. Appendices 1 1.2.2. On-call telephone assistance between the hours of 0700hrs to 1700hrs assistance, providing the information needed to help diagnose system hardware problems. 1.2.3. Provide substitute serviceable items to replace defective or inoperable items, within a 15-day period, shipped by common carrier. Emergency part service is ?next day?. The serviceable item will be guaranteed operable for 90 days from installation. Robins Air Force Base personnel will install the serviceable item. 1.2.4. Provide On-site maintenance assistance and emergency repairs upon request by government, when government fails to repair malfunctions during routine maintenance. Contractor shall respond and schedule site visit for routine maintenance request within 2 days from notification. For emergency responses the contractor shall arrive on-site within 24 hrs from notification of malfunction. The dates for on-site service shall be coordinated and scheduled with the Government points of contacts. 1.2.5. Provide scheduled maintenance on each listed test station as defined by the Original Equipment Manufacturer (OEM). 1.2.6. Entries will be made in Government provided logbook. The Government will maintain custody of the system log. Contractor representative shall make counter entries in the log when on-site, notating name, date and time of arrival/departure with a description of the repair or work done to the system. Downtime begins when the government notifies the contractor. 1.3. Software Maintenance 1.3.1. Provide software licenses and updates as required for software listed in Section 5, Appendix 2. 1.3.2. Correct software errors and defects as required, ensuring system performance is in accordance with the selected contractor?s published specifications. 1.3.3. The software updates shall be delivered in a format compatible with the Operating Systems installed on the hosts Teradyne Test Stations outlined in Section 5. Appendices 2. The software shall be delivered on compatible media for the host testers. 1.3.4 The contractor shall provide on-call assistance for user problems and software problems. Response time shall be within 2 hours after notification from the Government. 1.3.5 The contractor will provide the comprehensive documentation required for installation and utilization of these software /updates for each set of software supplied under this contract. This documentation is to include all standard commercial documentation, including system manuals, user?s manuals and desktop reference material. 2.0 SERVICES DELIVERY SUMMARY Performance Objective PWS ref Performance Threshold Provide replacement parts 1.2.3 Routine parts arrive within 15 daysParts for emergency arrive within 24 hours Provide on-site routine and emergency assistance for repairs 1.2.4 Routine maintenance scheduled within 2 days of notificationEmergency maintenance within 24 hours Provide on-call software assistance 1.3.4 Within 2 hours of notification 3.0 GOVERNMENT-FURNISHED PROPERTY AND SERVICES 3.1 GOVERNMENT FURNISHED FACILITIES. The Government will furnish work space to the contractor if needed. 3.2 GOVERNMENT FURNISHED EQUIPMENT AND SUPPLIES. The government will furnish access to all test stations and telephones for use with assigned work and office supplies incidental to assigned work. Government will provide a system log with double entries to the contractor. The government will document all changes, both temporary and permanent to the system equipment. 3.3 RECORDS, FILES, DOCUMENTS, AND WORK PAPERS. All records, files, documents and work papers provided by the Government or generated in support of this effort are Government property, it shall be marked, maintained and disposed IAW FAR 3.104 and AFI 37-131. 4.0 GENERAL INFORMATION 4.1 Mission Essential Services: Contractor services provided under this contract are designated as non- mission essential IAW DoDI 3020.37. The contractor is to be aware of the discontinuance of PWS requirements during a crisis situation. 4.2 Quality 4.2.1 QUALITY ASSURANCE PERSONNEL (QAP). The Government Functional Commander / Function Director (FC/FD) will designate a primary and an alternate (QAP) as a single point of contact through which all contractor-provided services will be requested and/or accomplished. The QAP shall review the contractor?s performance in accordance with the performance plan. The government will notify the contractor, in writing, of any changes of the QAP or alternates through the Administrative Contracting Officer within five workdays of the change. The alternates may perform all functions of the (QAP). 4.3 Reports: 4.3.1. The contractor shall submit a Monthly Status Report (MSR) to the Quality Assurance Personnel (QAP) assigned to the contract or his/her authorized alternative. Monthly status reports will be used by the QAP to evaluate performance of contractor IAW the Performance Plan. Upon acceptance of the MSR by the QAP with his/her signature, the MSR will be used to authorize payment of the contractor?s monthly billing invoice. MSRs may be submitted electronically using MS Word or Adobe PDF formats. The report shall include, as a minimum, the following: Period of performance, date of incidents, incident reports, serial no. of equipment serviced, corrective actions, parts removed/replaced, operational status of test station or ancillary equipment and comments. The monthly status reports to the QAP representative shall be submitted by the fifteenth (15) of each month, beginning second month of contract award date. 4.4 HOURS OF OPERATION: 4.4.1 NORMAL HOURS OF OPERATION Contractor shall schedule hours of work from Monday through Friday, between 7:00 am and 5:00 pm 4.4.2 HOLIDAYS. Government holidays and closings, such as energy conservation days, observed by WR-ALC general civilian populace will be observed. Contractor work schedule will conform to Government Holidays. 5.
