Description
This announcment consists of technical questions and answers regarding the requirment set forth in the solicitation: Technical Questions/Answers Regarding Solicitation 541-068-06 Q1. What is the size and/or dimensions of the room which requires remediation? A1. 4, 050 sq.f. (+/- 75%) Q2. Is there a specific secure facility designated off-site for the work? If so, what is the distance between the room housing the documents and the pre-determined secure facility? Additionally, what parameters are in place to securely move the contaminated documents out of the VAMC without contaminating any other part of the hospital? If not, what security is needed at the off-site facility? A2. The secure cleaning facility is now located on site and is the area located directly above the contaminated records in the same room (H27). Q3. There is no provision within the solicitation regarding decontaminating the offsite secure facility. Do you want this factored into the hourly rate, or as a separate line item? A3. The secure cleaning facility is now located on site and is the area located directly above the contaminated records in the same room (H27). Q4. Are there any transportation requirements/constraints regarding this solicitation? How do you want the transportation priced out? A4. Since all decontamination will be done in the same room, there are now no transportation needs. Q5. What is the estimated total amount of documents contained within the room? Since all documents must be removed, is there an estimated weight or volume you can associate to this solicitation? A5. Approximately 18 full rows of file cabinets and 12 rows with six sections (file cabinets) each section is approximately 36? and has six shelves. There is also a large number of documents located in baskets and on the floor and therefore there is no adequate way to estimate. Q6. What is the average number of pages per folder? What is the average number of pages with data on both sides? A6. Folders can vary from having 3? of paper files to an envelope containing a single document. Therefore, there is no adequate way to estimate. Q7. Have all moisture/water issues in room H27 been rectified or will this be part of the remediation effort? A7. All moisture/water issues in room H27 have been rectified. Q8. Will access to the patient records by staff of LSVAMC be necessary while they are in the secure cleaning facility in North East Ohio? A8. Access will only be necessary for a minimal number of medical center staff. (i.e. Chief of Engineering, Chief of Bio-Medical Engineering, the Contracting Officer, Chief of Patient Care Administrative Services, Chief of Acquisitions and Material Management.) The secure cleaning facility is now located on site and is the area located directly above the contaminated records in the same room (H27). Q9. Are there any restrictions regarding access to room H27 or the secure cleaning facility? A9. There is one entrance and exit into room H27. Q10. Will LSVAMC be supplying the document boxes and sealing material or should this be included in the price of the proposal? A10. The medical facility will not be supplying boxes or sealing material and the contractor should therefore include the materials in the price of the proposal. Q11. Was the solicitation originally a Hub Zone solicitation? A11. No Q12. There is no reference to specifications for the Technical Response, please advise of any page limits, font requirements, format style, and or binding requirements. A12. There are no restrictions. Q13. What is the delivery method for this response? A13. Email or fax. Response instructions are listed in the solicitation. Q14. Does the interior of the HVAC duct system have to be cleaned? A14. No. Only exterior HVAC must be cleaned. Q15. What is the performance period from project start to finish? A15. A meeting will take place after the award has been made in order stipulate the appropriate performance period for the project. Q16. Are there set hours of operation regarding this solicitation? A16. As of now, remediation will occur during normal business hours. 8:00 am to 4:30 pm until a change in time is stipulated by VA personnel or there is a hindrance upon patient care in that time frame. Q17. Has the inventory protocol been established? If so, what are the parameters required for tracking? No inventory process is necessary. All documents will be cleaned and boxed by the contractor. Q18. Will only documents with visible mold be treated? A18. NO. All Documents will be remediated page by page and all covers. Q19. Is there a need to access the patient files while the documents are being cleaned? A19. No, unless it is an absolute emergency. Q20. What if number of documents to be cleaned is over 10%? A20. All documents will be cleaned page by page and all covers. Q21. Do the lighting fixtures have to be opened up and the bulbs and interior cleaned? A21. No Q22. Does the 2nd mechanical storage room, through the double doors, have to be cleaned? A22. No, However, any precautions that may be set forth to eliminate transport of spores into the area should be taken. Q23. Does every page need to be cleaned in the files? A23. Yes Q24. Do the files need to be re-boxed or can they be re-shelved, after the area has been cleared? They must be boxed. Q25. What is the clearance criterion? A25. Each contractor must provide a detailed clearance process within their proposal in order to ensure that VA personnel can adequately deduce that the remediation is 100% successful. Q26. Can the negative air machines be vented to the outside? A26. At this juncture, no.
