Description
RFP # 666-00577-06 Artificial Limbs The Department of Veterans Affairs (VA), VISN 11 (Michigan, Indiana, and Central Illinois) has an ongoing program to furnish artificial limbs, to those veterans that require them and are eligible for assistance. It is VA?s intention to solicit proposals and award multiple contracts for each VA Medical Center (VAMC) under the authority of Title 38 U.S.C. Section 8123 for 1-Base Year with 4-Option Years. Contractor?s facilities must be within 80 miles of the particular VA medical center they are proposing on (www.mapquest.com will be used to determine distances). On a VISN (Veterans Integrated Service Network) basis for fiscal year 2005, VA spent an average of $6064.00, each for the purchase of 330 new artificial legs. During that same time period, VA recorded incidences of repair to artificial legs with an average cost of $ 1,257 per incident and 30 incidences of repair to artificial arms with an average cost of $ 638.00 per incident. For fabrication and repair of artificial limbs, the contractor?s prices proposed shall offer a percent discount off the established L-Code prices applicable to the geographic area at the time of performance: EXAMPLE: HCPC Code: L5620, Description: Test Socket below knee Current L Code pricing: $236.53, Contract Discount: 10% Total Cost: $212.88. Standards of Performance The Contractor shall fabricate all prosthesis in strict conformance to the prosthetic prescription that has been provided by authorized VISN 11 personnel. Contractors shall not add or substitute components or alter the beneficiaries? prescription in any way without prior written approval from the VISN 11 person who issued the prescription. Educational and Training Requirements All contractors shall have a full-time American Board Certified (ABC), or Board for Orthotist-Prosthetist Certification (BOC), or State licensed prosthetist on staff at each of their locations. Each location is required to maintain lab accreditation with ABC and/or BOC. Approved Facilities The contractor?s facilities shall be approved by the Facility Accreditation Program of the American Board for Certification in Orthotics and Prosthetics, Inc. (ABC), the Accredited Facilities Program of the Board of Certification (BOC), VA reserves the right to inspect contractor?s facilities.
