Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ). The solicitation and incorporated provisions are those in effect through Federal Acquisition Circular 2003-23. This procurement is a Total Small Business set-aside. NAICS code is 423510. Small business size standard is 100 employees. Requirements are as listed: SUPPLIES AND PRICES: (1) Corrugated Polyethylene Culvert ? Dual Wall (Corrugated Exterior, Smooth Interior); 24-inch Diameter, 30 Foot Length, Each, Quantity - 6; PLUS 5 Coupling Bands (3) Corrugated Polyethylene Culvert ? Dual Wall (Corrugated Exterior, Smooth Interior); 24-inch Diameter, 20 Foot Length, Each, Quantity ?8; PLUS 6 Coupling Bands. (4) Corrugated Polyethylene Culvert ? DESCRIPTION: The work to be done under these specifications consists of furnishing and delivery of the supplies listed as specified herein. DELIVERY: Items 0001 - 0004 to be supplied under this purchase order shall be delivered F.O.B. Destination to the Snyder Work Center,1/4 mile off US Highway 101, approximately 18 miles NE of Forks, WA. Items 0005-0009 to be supplied under this purchase order shall be delivered F.O.B. Destination to the Norwood storage area, 3 miles beyond the Quinault Ranger Station on the South Shore Road of Lake Quinault, WA (a total of 5.5 miles from US Highway 101). Delivery shall be by a truck with a flatbed or flatbed trailer (enclosed conveyance is not acceptable). Delivery shall be within 20 days of Contractor receiving purchase order. Items shall be unloaded as directed by the Forest Service representative. Contractor is responsible for unloading delivered items. Prior to shipment, the Contractor shall provide 48 hours advance notice of delivery date to the Government by contacting: Chuck McDonnell (360) 288-0242, as the primary contact. Secondary contact: Art Gibson, (360 765-2212 MEASUREMENT AND PAYMENT: Payment will be made at the purchase order unit price for the quantity of acceptable material delivered. No payment shall be made for supplies damaged because of Contractor fault or negligence. INVOICES: Invoice shall be sent to the following address: Acquisition Management, Gifford Pinchot National Forest, 10600 N.E 51st Circle, Vancouver, WA 98682-5419. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. The provision FAR 52.212-2, Evaluation - Commercial Items, will be used. Offers will be evaluated on price and past performance. The two factors are of equal importance. Offerors should complete and include a copy of the provision FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with the offer. Clause 52.212-4, Contract Terms and Conditions - Commercial Items, and Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition.
