- The Contractor shall provide 24 hour, seven days a week (to include all Federal Holidays) non-emergent transportation services for the beneficiaries of the DVA Medical Center, 800 Zorn Avenue, Louisville, KY 40204 and it's Community Based Outpatient Clinics. Care shall be provided during the transfer of a patient according to the orders of the attending physician. This procurement is a Total Small Business Set Aside. It is a Commercial Item procurement under 52.212-1 Instructions to Offerors, 52.212-2 Evaluations, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders.
