- This is a combined synopsis/solicitation for gym equipment maintenance (F2Q3306102A100) issued on 27 Apr 06 prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation will be issued on the World Wide Web and can be found at the following web site: http://www.eps.gov/. This solicitation is being issued as a Request for Quote (RFQ) using Simplified Acquisition Procedures under the test program for certain commercial items found at FAR Part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-07 and Defense Federal Acquisition Regulation Supplement, Defense Chang Notice (DCN) 20060223. The NAICS code is 811310 and the small business size standard is $6,500,000. Attached below is the Statement of Work and Requirements for this solicitation. Place of delivery for acceptance and FOB point will be 36 Sweeney Blvd, Langley AFB, VA 23665. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable. The contract will be for one (1) base year and three (4) option years. This acquisition is a 100% Small Business Set Aside IAW 19.502-2(a). (a) The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offeror conforming to the Statement of Work in this solicitation will be most advantageous to the and can meet the Government?s Period of Performance. Quotes are due and shall be received by 8 May 2006 and shall be received at 1 CONS, 74 Nealy Avenue, LAFB, Va 23665 not later that 3:00 P.M. Day Light Saving Time. Fax submission will be accepted at 1-757-764-7447 or e-mail Contract Specialist at scott.deloera@langley.af.mil. It is the offeror?s responsibility to ensure receipt of fax or e-mail quotes. Quotes shall include: (1) letter signed by an individual authorized to bind the company with a schedule (list) of offered items to include unit and total price. Completed Representations and Certifications. Offerors not submitting sufficient information may be eliminated from competition. Questions concerning this acquisition shall be submitted in writing and e-mailed to the address provided . No paper copies of Request for Quote will be provided. STATEMENT OF WORK-EQUIPMENT MAINTENANCE 1. Description of work. a. Scope: The contractor will perform the following services to approx 330 pieces of exercise equipment. The contractor shall provide all personnel, tools, parts, supervision, and other items necessary to inspect, and repair equipment at Langley Air Force Base Fitness Centers as defined in this statement of work. The equipment in the Fitness Centers will be updated on a yearly basis as new equipment comes in. b. Repairs: The Fitness Center Director or designated representative has the authority to approve repairs. All replacement parts required to maintain equipment will be ordered 24 hours after the initial service call. 2. Personnel/Qualifications. a. Service technicians: All technicians must possess a minimum of specialized experience on the repair and maintenance of commercial brand name fitness equipment to include but not limited to Life Fitness, Cybex, Stair Master, Nautilus, Body Master, Paramount, Hammer Strength, Precor, Star Trac, Versa Climber, Nordic Track, and Cycle Reebok. The only exception shall be a trainee technician who is under the direct and constant supervision of a qualified technician as outlined above (see attached equipment listing). b. Contractor primary POC or manager: The contractor shall provide an individual to serve as the contract manager. The contract manager is responsible for any and all work preformed. The name of this person and an alternate, if any shall be provided to the Fitness Center Director within ten days of the contract award date. c. Employees: The government is authorized to restrict or demand removal of employee?s under this contract that are identified as a potential threat to the health, safety, security, general well being, or operation, mission of these facilities and or its patrons. 3. Quality Control. a. The contractor shall ensure that all work is performed under this statement of work meets the quality outlined in paragraph 7. 4. Hours of Operation a. The contractor shall be available for all work to be done on equipment between the hours of 0800 to 1700, Monday through Friday for both facilities. b. Any work performed after these hours shall be given coordinated at least 24 hour in advance with the Fitness Center Director and only at no addition cost to the government. c. The contractor shall coordinate and complete all eight (8) preventive maintenance visits or about by the 28th of every month based on the manufacturers recommended frequency of maintenance requirements. If maintenance cannot be performed in this manner, the contractor will notify the fitness center director in advance. If these days fall on a weekend or holiday, work will be preformed on Friday. 5. Government Furnished Supplies and Services. a. An individual from the using activity is required to sign all service invoices verifying completion of work and satisfaction of services received. b. The using activity has the right to supply the contractor with any parts on hand that would bring a piece of equipment up to standards. 6. Contractor Furnished Supplies and Service. a. Replacement Parts: The contractor shall provide all parts unless parts are available from the using activity. All replacement parts shall be new factory warranty parts; the Fitness Center Director shall approve any rebuilt parts in advance. b. Upholstery: The contractor shall provide all materials and equipment necessary to replace or repair any covering on equipment as necessary. The covering must be within the specifications of the manufacturer and match the color of the existing upholstery or be approved by the Fitness Center Director. c. Maintenance Repair Log: The contractor shall provide and maintain a detailed listing of any and all service performed on each individual piece of equipment covered under the agreement. The log must include the following information: manufacturer, model number, serial number, unit number, date work performed, description of work performed, date of service call, date repaired, cost of repair, parts installed. 7. Specific Tasks. a. The contractor shall provide scheduled preventive maintenance and on-site unscheduled service calls in accordance with the manufacturer?s specifications on all equipment housed at the Fitness Center. Within 15 calendar days after award of the contract the contractor will submit a schedule of manufacturer recommended preventive maintenance frequencies to the contracting officer and to the fitness center director. The fitness center director will review and recommend acceptance of the schedule by notifying the contracting squadron POC in writing. b. Preventive maintenance is the care and servicing of equipment for the purpose of retaining the equipment in servable condition in accordance with manufacturer?s specifications as listed in technical literature. This inspection includes checking for proper operation, detection, correction of machine failures, before they develop into major problems. Maintenance includes, but is not limited to, inspection, lubrication, adjustment, calibration, interior cleaning, pad repair, and replacement of worn and /or deteriorated parts. Preventative maintenance should be accomplished within the prescribed time frame and recommended by the manufacturer. c. Upon arrival the contractor service technician will work continuously until repairs are completed, per manufacturer?s specifications. The only exception is delay due to shipping of replacement parts. d. The contractor shall respond to unscheduled service calls within 24 hours of notification. e. The contractor?s service technician shall prepare a service report for each service call (schedule or unscheduled) and obtain a signature from the using activity to certify that the service was completed and accepted. f. Before replacing parts, the contractor shall provide the Fitness Center Director a list of parts required for the job and the cost of those parts. 8. Points of Contact (POC). a. The Fitness Center Director or designated individual can be contacted at 225-8158/8165/8166/8167. 9. Reliability. a. Uptime is defined as the time the equipment is performing according to the manufacturer?s specifications. Downtime is defined as the time when the system is deemed not available due to breakdown, ongoing corrective maintenance, or failure to perform according to specifications. 10. Serviceable Equipment. a. Only equipment in serviceable condition is to be covered under this agreement. b. Equipment is considered serviceable only if it is performing all of its intended functions can be repaired at a lower cost than it can be replaced. The contractor will within 15 calendar days after award of the contract conduct with the fitness center director a survey of equipment for the purpose of determining the serviceable condition and development of the manufacturers recommended maintenance frequency mentioned above. c. Within ten business days after receipt of this order, the contractor shall notify the POC and inspect all equipment to be maintained. 11. Pricing. a. Prices to the government shall be as low as or lower than those charged to the contractors most favored customer, in addition to any prompt payment discount. 12. Services Not Included. a. Electrical work exterior to the machine. b. Excessive repairs necessitated by accident, fire, abuse, or misuse by government personnel as determined by the POC, without express written consent by same. 13. Invoicing. Contractor shall bill monthly for maintenance, quarterly maintenance, unscheduled maintenance labor, repair labor, and parts. Invoices shall include contract purchase order, billing period, amount, make, model, and serial number of each machine serviced and replacement parts ordered.
