- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-06-Q-0225 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-07. (iv) This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code 332311 and small business size standard 500 Employees. (v) The contractor shall provide the following items: Line item 0001 ? Storage Lockers (24 each), no part number, lockers are custom-made. 8'-4" X 4'-4" X 8'-5" high, four-sided enclosure, 4'-0" sliding gate on one side 8' side with padlock lug, three shelves 2'-6" evenly spaced opposite sliding gate, one hanger bar approximately 1-10" long. Ceiling panel included, six rows of four lockers, solid sheet metal backs on two rows (end rows), color: gray. Manufacturer: Wirecrafters or equal. Line item 0002 ? Alert cages (2 each), no part number, lockers are custom made. Alert cages - 2 X 7'4"L X 9'6"W X 9'0"H, color: gray. Manufacturer: Wirecrafters or equal. Line item 0003 ? installation (1 lot). The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (x) Offerors must include a completed copy of the provision at, 52.204-8, Annual Representations and Certifications and submit with its offer. (vii) Clause 52.204-9, Personal Identity Verification of Contractor Personnel ? contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
