Description
This is a combined synopsis/solicitation for the purchase of commercial items, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only so licitation. Quotes are being requested and a written solicitation will not be issued. Solicitation number W911S8-06-Q-0016 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-08 and DCN 20060412. This requirement is an unrestricted solicitation. The description of the commercial item is for 50 computer roll stands to be used in a major medical center emergency room facility. The statement of work is as follows: CLIN 0001. Computer roll stands will provide automation system mobility and convenience of use in the emergency room environment. They provide a unique and flexible means to have networked computer technology available at the patient bedside. The roll s tand must have the capability to raise and lower the work surface i.e. keyboard, mouse, and display to allow the user to be either seated (29 inch work surface height) or to be standing (39 inch work surface height). This surface height adjustment must be conveniently adjustable to the user with control device such as foot pedal release mechanism and hydraulic lift. The base must be of a minimum five wheel caster arrangement capable of supporting 50 lbs with at least two casters being of a locking type. The roll stands must have platforms for CPU mounting in a vertical position with clamping device to secure it from dislodging. A mounting capability for a flat panel display (TFT) of minimum 15-inch display that can be easily adjustable vertically, latera lly and for tilting. This display mounting adjustment feature should have padded grips on the sides to provide comfort, durability and also serve as bumpers to help protect the display and adjacent devices or obstacles. A keyboard tray that will allow sp ace for standard keyboard with the ergonomic design for wrist support, protective side bumpers, and support for mouse device for left or right handed users. Will also require a secure docking device for the mouse when cart is in a transport mode. Comput er roll stand will also require features such as padded handle to ease of movement of portable device while ensuring safety to the user and providing the appropriate element for handling the roll stand so as not to bend or break ancillary features. The co mputer roll stands require a separate horizontal work surface area of at least 12 inches by 16 inches that would have raised edges to prevent small items from rolling off and swivel capability. The computer roll stands require a vertical work surface wit h a swivel capability and designed for securing documents for hands free review. A minimum four-outlet hospital-grade power strip along with a fixed power cord management device will be required. The computer roll stand should be expandable for future ad d-on accessories or devices. The computer roll stand should be designed adequately to withstand the rigors of the hospital environment and be esthetically pleasing. Must be made of an aluminum framework, painted, glass bead, or stainless surfaces with th e ability to maintain a microbial resistant surface that will support sterilization cleaning processes. Must have a minimum 18 degree tip ratio for staff and patient safety. All 50 computer roll stands will be delivered to the Madigan Army Medical Center, Tacoma, WA 98431. Acceptance will be at the destination. FOB point is destination. The selection resulting from this request for quotations will be made on the basis of the lowest priced, technically acceptable quote from a responsible quoter with acceptable or neutral past performance. Once the Contracting Officer has decided to whom a n order will be issued , a unilateral order will be issued using a SF 1449. A purchase order will be in existence once the Contracting Officer has signed the order. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offeror, whenever it appears, shall be replaced with the term quoter. Also, paragraphs (g) and (h) of FAR 52.212-1 are deleted. Local Fort Lewis clauses 52.111-4017, Government-Furnished Identification Badges, 52.111-4133, Commercial Vehicle Access to Fort Lewis, and 52.111-4004, Electronic Commerce, apply to this acquisition. Local clause 52.111-4017 states: Any Contractor empl oyee (including subcontractors at any tier) who requires access to Fort Lewis to perform work under this contract shall obtain a Government-furnished identification (ID) badge. a. The Contractor shall, prior to the contract start date, provide the COR w ith the name of each employee who shall require access to Fort Lewis to perform work. This notification shall include the employees last name, first name and middle initial and the employees Social Security Number. The contractor shall ensure that the employee obtains a Government-furnished identification badge prior to beginning performance. Contractor employees hired during the term of the contract must obtain the badge before beginning work under the contract. If an employee no longer needs a badge for any reason (e.g., quits his/her job or no longer performs work under the contract), the Contractor shall return the badge to the COR within two (2) calendar days of such change. If the badge cannot be returned within the required time frame for any r eason, the Contractor shall immediately notify the COR verbally, followed up in writing the next work day. An employees inability to obtain entrance to a Government installation because he/she does not have the required Government-furnished ID badge shal l not excuse timely performance of the requirements of this contract. Contractor employees may be denied entry to the Installation by Military Police if it is determined that such entry may be contrary to good order, discipline, or the security of the Ins tallation. The Government may change the location(s) at which badges are issued or returned, with or without advance notice to the Contractor, at no additional cost to the Government as a result thereof. Employees who only require intermittent access to a Government installation for performance of this contract may be required to obtain a visitors pass rather than be issued an ID badge. Such determinations will be at the sole discretion of the Contracting Officer. b. Government-furnished ID badges shal l not be reproduced or copied by the Contractor, its subcontractors, or their employees. If an employees badge is lost or stolen the Contractor shall verbally report the loss or theft to the COR on the day such loss or theft is discovered; followed by a written report of the circumstances to the Contracting Officer. The written report shall be completed one (1) calendar day after the loss or theft is discovered. c. The Government-furnished ID badge shall not be used for access to any Government installat ion except for performance of work under the contract for which it was issued. d. The Contractor shall, upon expiration or termination of the contract, collect all Government-furnished ID badges and turn them in to the COR. The final invoice will not be c onsidered proper for purposes of the Prompt Payment Act (FAR 52.232-25, in Section I of award) until all Government-furnished ID badges have been accounted for. Local clause 52.111-4133 states: Procedures for commercial vehicle access to Fort Lewis are subject to change without prior notice. Current access information may be obtained by calling 1-877-RAPIDGATE (1-877-727-4342). The following procedures apply if the Fast Access Gate Program is terminated or expires. See the clause en titled Commercial Vehicle Access To Fort Lewis  Fast Access Gate Program. Commercial vehicle access to Fort Lewis will be allowed only at the Logistics Center Gate (Exit 123 from I-5) and the D Street Gate. The Logistics Center Gate is open 24 hours a da y, 7 days a week (to include federal holidays). The D Street Gate is open Monday through Friday, excluding federal holidays, from 0530-2130. These gates are open for inbound commercial vehicle access and inspection. In the event both the Logistics Cent er Gate and the D Street Gate are closed, commercial vehicles must use the Main Gate. All commercial vehicles will be searched. A visitor pass must be obtained. The Contractor should anticipate delays in getting commercial vehicles on post. The Contract or must also allow additional time for commercial vehicles to reach their destination by driving through Fort Lewis. If the commercial vehicle is carrying a load of cement concrete or hot asphalt concrete for delivery, the driver shall notify the gate guar d as soon as possible and request that the vehicle be given priority for being searched; however, the Government does not guarantee that the vehicle will be given priority. On weekends large vehicles (needing greater than 12ft-5in clearance) will require a time stamped searched label to gain access to North Fort Lewis. Searched labels will be issued at the Main Gate, as appropriate. Drivers needing access to North Fort Lewis must inform the gate guard that their vehicle is over 12ft-5in in height and that they will require access to North Fort Lewis. The driver will receive a briefing on proper procedures and a searched label. The Contractor shall ensure that its drivers, including drivers of subcontractors at any tier, comply with the procedures a s explained to them for access to North Fort Lewis. Local clause 52.111-4004 states: In accordance with the FAR Part Two electronic commerce definition and FAR Subparagraph 4.502 (Electronic Commerce in Contracting Policy), the Contractor shall communi cate with the Government utilizing electronic mail. Quoters are required to submit completed copies of the provisions at FAR 52.212-3 and its ALT I, Offeror Representations and Certifications-Commercial Items, and DFARS 252.212-7000, Offeror Representations and Certifications - Commercial Items with their q uotes. Online submissions through the ORCA website (http://orca.bpn.gov) are acceptable. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-3, 52.222-19, 52.222-3 6 and 52.232-33. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items is applicable, to include under paragraph (b): 252.225-7001 (applicable only if the items are liste d in FAR 25.104(a)) and 252.232-7003. FAR provision 52.252-1 is included in this solicitation.
