- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SOLE SOURCE TO SIERRA ROACK Who is currently working on site this is follow on work to previous contract W912Dw-05-P-0271 SECTION 01000 STATEMENT OF WORK CONCRETE & ASPHALT CRUSHING MALMSTROM AFB GREAT FALLS, MONTANA (CASCADE COUNTY) 21 April 2006 I. DESCRIPTION OF WORK: The contractor shall provide all labor, materials, equipment, transportation, and apparatuses required for the crushing (recycling) of concrete and asphalt. The project site is located at Malmstrom Air Force Base, Great Falls, Mon tana. A. BID ITEM 0001: 1. The contractor shall provide all labor; materials, equipment, and apparatuses to crush (recycle) approximately 3,245 tons of concrete to 1 ? minus material. 2. The specified gradation requirements shall apply to the completed base course. The aggregates shall have a maximum size of 1 1/2 inch and shall be continuously well graded within the limits specified in TABLE 1. Sieves shall conform to ASTM E 11. Any crushed material which does not comply with one of the following gradations shall be stockpiled separately. Sieve Analysis shall be performed by the contractor for every 1000 tons, or portion thereof. TABLE I. Material shall be weighed and stockpiled on site. Contractor shall remove all reinforcing steel and wire mesh from crushed material. The materials to be crushed and blended are shown below. II. DRAWINGS & SPECIFICATIONS A. N/A III. CONSTRUCTION PHASING AND RESTRICTIONS A. The Contractor shall observe all local, state, federal, and Malmstrom AFB regulations while performing this contract. All construction shall meet or exceed applicable industry standards. B. The contractor shall be responsible for obtaining any permits, approvals, required licenses, and base passes required to perform the work. C. U.S. Army Corps of Engineers, Safety and Health Requirements Manual, EM 385-1-1 shall be considered a part of this contract and will be enforced as such. A site-specific safety plan shall be submitted for approval prior to the start of construction. D. The contractor shall attend a pre-construction meeting presided by the C.O.E. Project Lead prior to commencing construction activities. The contractor shall contact the C.O.E. Project Lead within 5-days of issuance of contract NTP to establish a date a nd time for the pre-work meeting. E. The contractor shall field verify all quantities by weighing crushed material, using a scale integral to the crushing equipment, a portable truck scale, or the base truck scale. The contractor will coordinate daily with the COE representative to take s tarting and ending readings on any portable scale equipment used at the jobsite. If portable (jobsite) scale is not available, the contractor will have to truck the material to the base scale for weighing prior to crushing. Contractor shall submit weight slips for payment. IV. CONSTRUCTION FACILITIES AND TEMPORARY CONTROLS A. The Contractor is responsible for security of his own property and security of Government property when construction activities affect existing security measures. Note: No water or power connect ions are available at the crushing site. V. SUBMITTALS A. Required as per the Statement of Work. VI. CONTRACT COMPLETION REQUIREMENTS A. Required as per the Statement of Work and Specifications. VII. PERFORMANCE PERIOD A. The Contractor shall be required to commence work under this contract within 10 calendar days after the date the Contractor receives the notice to proceed and complete all work under this contract, including final cleanup of the premises, within 30 cale ndar days after the Notice to Proceed.
