Description
This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI-PB(HG)-2006-174-1-CDM, and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2001-27. The North American Industry Classification System (NAICS) is 423990, and size standard 100 employees are used for this requirement. This is a small business set-aside requirement. This acquisition is being conducted under simplified acquisition procedures in accordance with the provisions of FAR Part 13. The National Heart, Lung, and Blood Institute (NHLBI) Consolidated Operations Acquisition Centers (COAC) has a requirement for the National Human Genome Research Institute (NHGRI) of the National Institutes of Health (NIH) to purchase a Virtual Environment System with two state-of-the-art Immersive virtual environment technology systems. This Integrated system shall be made with the following components: 2 (two) each, nVisor SX HMD High resolution head mounted display manufactured by NVIS, 11495 Sunset Hills Road, Reston, Virginia 20190; 2 (two) each, PPT X4 (4 sensor) precision position tracker, manufactured by WorldViz LLC, 29 W. Anapamu, Santa Barbara, California 93101; three (3) each, Inertia Cube2 for head Precision orientation tracking system manufactured by Inter sense, 36 Crosby Drive, Bedford, Massachusetts 01730; one (1) each, 5DT Data Glove 4 Ultra (Right), VR peripheral - 5 sensor data glove, manufactured by 5DT, 15375 Barranca PKWY, G-103, Irvine, California 92618; two (2) each, NVISOR SX integrated Boom Microphone for HMDs; one (1) each, onsite setup service and a minimum of three (3) years support package including parts replacement warranty for two (2) tracking systems. This system shall allow researchers to investigate complex, multifaceted human behaviors and to perform innovative research and training in a variety of health-related domains. The system shall be delivered as negotiated with the successful contractor closed to the opening and operational of the new laboratory, which is expected to be by September 2006. The provisions of FAR Clause 52.212-1, Instructions to Offeror-Commercial Items, apply to this acquisition. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items is applicable to this acquisition. The quotations will be evaluated base upon the offerors demonstration of meeting the specific requirements requested on this synopsis. The award will be made on the basis of documented successful past performance obtained from references provided by the offeror on identical system integration , other relevant past performance information obtained from other sources known to the Government, the technical specifications, and price. The offerors must submit written descriptive literature of the components identified in their proposal to integrate the system, and an itemized price list and warranties. The Government intends to evaluate proposals submitted through contracting by negotiation procedure for this acquisition. The initial offer must contain the offerors best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action and waive informalities and minor irregularities in offers received. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addenda to the clause read as follows: The offeror must include in their quotation, the unit price, the list price, shipping and handling costs, agreement on the delivery time of the end product , the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. The Offeror must also be registered in the Government Contractor Registry System, available at http://WWW.ccr.gov. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions, applies to this acquisition. The clauses are available in full text at http://WWW.arnet.gov/far. Interested vendors capable of furnishing the government with the item specified in this synopsis should submit two (2) printed copies of a quotation to the below address or an electronic copy to moralesc@nhlbi.nih.gov. The quotation must reference solicitation number NHLBI-PB(HG)-2006-174-1-CDM. The Government intends to award a contract by May 19, 2006. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the National Heart, Lung, and Blood Institute, Contracts Operations Branch, Procurement Section, 6701 Rockledge Drive, Suite 6149, Bethesda, Maryland 20814-7902, Attention: Ms. Cecilia Morales. Fax quotations will only be accepted if dated and signed by an authorized company representative.
