Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Army Medical Research Acquisition Activity (USAMRAA) is issuing a 100% Small Business set aside (Note 1) on behalf of the Armed Forces Institute of Pathology (AFIP) located in Washington, D.C. for DNA Collection Kits. This combined synop sis/solicitation is a Request for Quote (RFQ) for a firm fixed price (FFP) contract for base year one 4 one-year options. Anticipated commencement is August 2006. The synopsis/solicitation documentation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 9-45. Release of synopsis/solicitation is scheduled on or about 11 May 2006 and will be distributed solely through General Services Administration Federal Business Opportunities (FedBizOpps) website http://www .fedbizopps.gov . Hard copies of the RFQ document will not be available. The site provides downloading instructions. All future information about this synopsis/solicitation, including amendments, will also be distributed solely through the FedBizOpps web site. Interested parties are responsible for monitoring the FedBizOpps website to ensure that they have the most up-to-date information about this synopsis/solicitation. DESCRIPTION and QUANITY: These kits consist of 19 items (contact Julie Burns-Feaga a t julie.burns@det.amedd.army.mil for a copy). REQUIREMENTS: The contractor shall be responsible for assembling individual DNA collection kits for shipment, provide warehouse and storage facilities, inventory and quality control, preservation and security of all products stored in the warehouse, provide delivery, freight, and shipping services for these kits. All interested parties will be required to submit a prototype of each component part and shall be submitted to Armed Forces DNA Identification Labora tory (AFDIL) for analysis and suitability determination of the component part for evaluation. Test samples shall be sent to: DoD DNA Registry, Armed Forces Institute of Pathology, Annex Bldg 101, 2nd Floor 1413 Research Blvd, Rockville, MD 20850. (See SO W, section F Implementation). Test samples SHALL be received by 8 June 2006. Offerors will be notified by electronic mail the week of 22 June 2006, regarding whether or not to submit a proposal based on government testing results. EVALUTION CRITERIA: (contact Julie Burns-Feaga at julie.burns@det.amedd.army.mil) CLAUSES and PROVISIONS: FAR 52.212-1, instructions to Offerors-Commercial Items (OCT 1995), FAR 52-212-2 Evalua tion-Commercial Items apply to this synopsis/solicitation. 52.212-3 Offeror Representation and Certification-Commercial Items applies to this synopsis/solicitation. The contractor shall return a completed copy of this provision with its quotation. A cop y of the provision may be obtained at http://www.arnet.gov/far/ . The resulting firm fixed price award will incorporate the requirements of the clause at FAR clause 52.212-4, Contract Terms and Conditions  Commercial Items. The clause 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders  Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity Code, DUNS number, and Tax Identification number. As of 1 October 2003, all Contractors both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov .
