- The James A. Haley Veterans Administration Hospital (VA) intends to negotiate and award a Commercial Item Firm-Fixed Priced Contract for Off Site (at Contractors facility/place of business) Radiologic Diagnostic Imaging Services [i.e. Magnetic Resonance Imaging (MRI) Exams], including personnel, equipment, and supplies. Mobile services will NOT be considered. Services shall be provided for eligible veteran beneficiaries as referred by authorized VA personnel from the Department of Veterans Affairs (DVA/VA), Brevard County VA Outpatient Clinic (OPC), located at 2900 Veterans Way, in Viera Florida, with written reports and computer disc provided within 48 hours of examination completion. Potential offerors shall be located within a 30 mile radius of the Brevard Clinic to minimize travel for veteran population. Potential offerors shall have a fully licensed/certified facility in accordance with all Federal, State, County/City, and Nuclear Regulatory Commission requirements, and meet or exceed JCAHO Accreditation Standards for subject industry, and shall provide written evidence there of. Personnel proposed to provide services under resulting contract shall have a current license in a State, Territory, or Commonwealth of the United States or the District of Columbia, and be board certified in radiologic technologies. Solicitation No. 673-62-06 is being issued as a Request For Proposal (RFP) and shall be referenced in all proposal correspondence. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-8. Contractor shall provide all labor, equipment, supplies/parts, materials, facilities, expertise, and supervision to carry out the Radiological Service requirements of the solicitation. Awardee shall provide evidence of proper insurance coverage as required and outlined in the solicitation. All solicitation amendments will be posted on the same web site/URL. Offerors are solely responsible for checking the web site regularly to obtain and respond to solicitation amendments. The size standard for North American Industry Classification System (NAICS) code 621512 is $12.5 Million. NOTE: This solicitation is 100% set-aside for SMALL BUSINESS. All qualified offerors/firms regularly established in subject business industry who believe they can satisfy this requirement are invited and encouraged to provide their proposal package. Offers shall provide pricing for ALL line items including options (All or None basis), which shall be evaluated for fair and reasonable pricing, completeness, technical competency/capability to perform services (including Quality Plan), and satisfactory past performance record. All clinical or other medical records of VA Beneficiaries treated by the contractor shall be forwarded to the VA and are expressly deemed Government Property. Proposals shall be submitted on an “All or None basis”, partial proposals shall be rejected. All offerors shall be available to perform services immediately. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. Award will be made utilizing the Simplified Acquisition Procedures outlined in FAR subpart 13 and a combination of FAR parts 12, 13, 15 & 17 award procedures, as applicable. Award is anticipated to be made without holding discussions. Contract award is subject to availability of fiscal year funding. Period of service will be from the date of award through 30 September 2006. Contract will have three (3) one year option periods to be exercised individually and at the Governments sole discretion. Potential Offerors are advised to thoroughly read and review all synopsis and solicitation documents. Questions regarding this procurement must be submitted in writing not later than 2:30 PM EST, 22 March 2006. Questions received after the date & time indicated above will not be guaranteed a response. No telephonic inquiries will be accepted, however vendors may inquire by phone as to receipt of questions. Proposals may be sent to the POC electronically, by fax, hand delivered, or by mail, however original signature is required on SF 1449. The Government is not responsible for the quality of fax transmittals received. Proposal must be submitted in writing to the issuing office, be in accordance with the proposal preparation addendum outlined in the Solicitation, and be timely. No web-based offers will be accepted. A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (March 2005) is required with your proposal. The following clauses and provisions apply to this solicitation and resulting award, plus addenda 52.212-1, 52.212-2, 52.212-4, 52.212-5; additional provisions/clauses are outlined in the solicitation. Clause(s) may be viewed in full text at http://www.arnet.gov/far. Offerors shall verify current registration with the Central Contractor Registration (CCR) system (see: http://www.ccr.gov). The subsequent contract is subject to the Health Insurance Portability and Accountability Act of 1996 (HIPAA), for which a Business Associate Agreement will be required. Numbered Note 1 applies.
