Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442761090948; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-09 and Defense DCN 20060412. This acquisition will be a 100% small business set-aside; the North American Industry Classification System (NAICS) code is 423450. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6530. The Standard Industrial Classification (SIC) is 5047. This request for quotation consists of the following Biodex item or its equal: Item 0001: Biodex Balance System SD, model number 950-300, 1 ea. Specifications required of ?equal? product include: 1.) a stable balance platform that provides a variety of skill/intensity levels; 2.) an adjustable feedback monitor that provides information to the patient during treatment; 3.) adjustable support handles for stabilization within reach of the patient. Print-out capabilities for feedback would be preferred but is not a requirement. The delivery location is Travis AFB, California. The required delivery date is 30 days after award. If you are unable to meet the required delivery date, specify the delivery time for consideration. Upon award the contractor will be required to coordinate delivery with the customer.
