Description
This is a combined synopsis/solicitation for a commercial item papered in accordance worth the format in FAR subpart 12.6 as a supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The reference number for this procurement is RFQ500-28-06 and incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 2005-08. The North America Industry Classification System Code (NAICS) is: 541990. The Stratton VA Medical Center Albany , NY has a requirement for the service of a qualified and competent elevator service and installation company to Furnish and install Elevator Door Operators such as (MCE (Motion Control Engineering), smart track door operators(or equivalent) on Otis elevators #1 thru #5 to improve door safety and performance. Otis elevators #8 and #10 will require the furnishing, installation and labor to replace the front and rear bearings on gearless machines. These parts must be compatible with the existing Otis Elevators. Detailed spec of the parts is required before installation. The parts are to include the manufactures warranty. The work is to be preformed during normal working hours; one elevator must be worked on at a time. An informal schedule of work is to be turned in with quote. The schedules are subject to negotiation and approval. All personal safety equipment must be worn at all times and the contractor must conform to all OSHA Safety Requirements. Contractor will provide a one year warranty for materials and workmanship from the date of installation and final inspection. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-2 Evaluation-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial .Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items. Offers including the pricing information are to be submitted on company letterhead and must include a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the Internet www.arnet.gov/ for the FAR. http://vaww1.va.gov/oamm/acquisitions/ars/policyreg/vaar/index.htm Technical/Descriptive Literature (Ref. FAR 52.214-21) shall include but not be limited to technical specifications, product pictures, product features, etc., and shall be submitted with the quote or the quote will be rejected as non-responsive. Technical acceptability will be determined by information submitted providing a description in sufficient detail to show that the product meets the government's requirements. The following factors, in descending order of importance, will be used to evaluate quotes: (1) determination of product acceptability (2) technical ability & experience of offeror in the area of ventilator management systems, including references to customers of existing systems, (3) delivery schedule and (4) the parts and installation services are to be priced as separated line items to meet the Government's requirement. The Government intends to evaluate the information received and award with discussions if needed.
