- Provide all personnel, equipment, and supplies necessary for temporary interim certified home health aid services for the John D. Dingell Department of Veterans Affairs Medical Center, 4646 John R. Street, MI 48201. Contractor shall perform these services for patients residing in Detroit and Wayne, Oakland, and Macomb Counties. The North American Industry Classification System (NAICS) for this procurement is 621610, Home Health Care Services. The size standard is 10 million average annual receipts for the past three years. Projected period of service is from date of award through estimated November 30, 2006. This is part of a multiple award contract. Request for Quote is now available on this website. The requests for quotes may also be submitted in writing to the John D. Dingell Department of Veterans Affairs Medical Center, Acquisition Management (001R-A), 4646 John R. Street, Detroit MI 48201 or via facsimile at (313) 576-1077. Telephone or internet requests will not be accepted. All requests must reference solicitation RFP 553-00610-06. Proposals are due by 4:00 pm, local time, May 31, 2006. Contractor shall meet the following minimum requirements and include substantiating documentation to be considered: In home services are available 365 days per year. RN Supervisors are available to supervise the Certified Home Health Aids during each shift. Certified Home Health Aids are available on a full-time basis. Demonstrate compliance with Official Accreditation Decision Reporting or other such accrediting agency standards. Must be State Certified Home Health Aid. Minimum one year successful experience in performing specific type services required under this contract. Interested contractors must submit a minimum of three references including company name, contact person, company address, phone, and fax numbers. Include the number of years similar services were performed for each company listed as a reference. Submit recent copies of certificates of insurance. **** See numbered note 1.
