Description: This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a ?Request for Quote? (RFQ). PERFORMANCE WORK STATEMENT (PWS) RAF FAIRFORD FAMILY SERVICES COORDINATOR 1. DESCRIPTION OF SERVICES/GENERAL INFORMATION: This Performance Work Statement (PWS) details the general requirement for a contractor to provide non-personal services as the Family Services Coordinator (FSC). The contractor shall administer the typical family services program found at larger installations such as loan closet, family attic, newcomers tea and base brochure/video programs. The current work site is the RAF Fairford Family Services Building, Glocestershire, United Kingdom. 1.1. Current hours of Operation are 0900 ? 1500, Monday to Friday. Contractor will be scheduled to work by the Family Life/Readiness Coordinator not to Exceed (NTE) 120 hours a month. Contractor?s work hours are scheduled at the convenience of the Government. Work hours are scheduled to satisfy the minimum needs of the Government. 1.2. The contractor will coordinate all leave and other absences in writing, with the 420 ABG?s servicing FSC liaison. This coordination will take place no later than 15 calendar days prior to the planned absence. Sick leave will be coordinated through the FSC Liaison Officer as needed. The contractor will not be paid for workdays missed unless a subcontractor satisfies the requirement. 1.2 QUALIFICATIONS: 1.2.1. Should have supervisory skills as documented in previous work history with a general knowledge of all aspects of Family Services, to include administrative procedures and customer service; or one year of experience as a FSC Program Manager. 1.2.2. Preferably a minimum of six months of Family Services experience to include: Base Relocation Brochures/Video Loan Program, the Loan Closet, and Layette Program. 1.2.3. Should have basic familiarity with FSC programs and information and referral procedures. 1.2.4. Must have at a minimum sufficient time on station to complete the four- month base period of performance and one full option year of the contract. 1.2.5. Shall have computer skills (e.g. prior work experience, college courses etc.), to include using PCs and such software as Microsoft Word, Excel, and Power Point. 1.3 RESPONSIBILITIES: 1.3. 1. The Contractor will be required to sign-in at the start of the duty day and sign-out at the end of the duty day on a form provided by the Family Life and Readiness Coordinator. This data will be collected each week by the Family Life and Readiness Coordinator. This time sheet will be used to calculate the monthly pay. 1.3.2.. The Contractor shall open and close the assigned facility during the appropriate assigned times unlocking/locking all activity use spaces and the office area. 1.3.3. The Contractor will perform a facility walk through inspection to ensure all equipment and facility utilities and other functions are operational, clean and otherwise ready for daily use. Discrepancies will be annotated and communicated to the Family Life/Readiness Coordinator either in writing or verbally. 1.3.4.. The contractor will become familiar with emergency medical notification procedures for patrons and be able to notify authorities by telephone when situations occur. 1.3.5 Contractor will submit invoices on a monthly basis for payment based on an hourly rate. (See 1.3.1) 1.3.6. Administer and coordinate the expanded Family Services Program to meet the needs of the 420 ABS commander and the local military community per AFI 36-3009, Family Support Center Program, and local guidance. 1.3.7. Provide assistance in a professional manner. Extend proper courtesy in all respects to all customers. Confidentiality cannot be given to customers; however, all assistance provided is covered under the Privacy Act. Questions on this can be directed to the Information and Referral Specialist/GSU Coordinator at the RAF Mildenhall FSC. 1.3.8. Recommend changes to the commander as required to maintain a viable program that continues to meet the needs of the personnel and family members employed at or living in the RAF Fairford area. 1.3.9. Be the point of contact for distribution of information and publicity flowing to the RAF Fairford community from the RAF Mildenhall FSC. This includes working closely with the RAF Mildenhall FSC staff to coordinate quarterly classes, seminars, and workshops offered by the RAF Mildenhall FSC at RAF Fairford. 1.3.10. Provide information to clients about local services and resources. Maintain information on the community to provide effective service delivery. (a.) Develop and maintain an up-to-date continuity binder on the program. (b.) Maintain the automated Standard Installation Topic Exchange Service, or SITES, database and coordinate with the RAF Mildenhall Relocation Assistance Program staff to update the SITES database. (c.) Prepare narrative and statistical reports on AF Form 2806 identifying program trends, usage, and accomplishments. Provide these reports to the RAF Mildenhall FSC information and Referral Specialist/GSU Coordinator by the last day of each quarter. (d.) Identify new volunteers; maintain volunteer files to ensure annotation of hours worked, training, awards received and amount of childcare services used. (e.) Assist all walk-in clients in completing necessary paperwork to check out items from the loan closet, ensuring all items are returned in a timely fashion and in serviceable condition. Provide Family Services Life and Readiness Coordinator with any replenishment items required to sustain the Loaner Closet. (f.) Provide timely inputs to the 420 ABS Commander and First Sergeant on nominations for the 420 ABS Volunteer of the Quarter and Volunteer of the Year programs. (g.) Provide support activities in the form of information, referral and liaison for the Key Spouse Program and the FSC core programs. (h.) Market the expanded Family Services Program to the RAF Fairford community via speaking at spouses' groups, commander's call, and other media. (i) Develop and obtain information, books, magazines, etc., for a reference library for volunteers and customers. (j). Maintains the Family Attic as directed by the Family Life and Readiness Coordinator 1.3.11. The contractor shall ensure basic cleanliness; safety and operational rules and procedures are maintained. 1.4. GENERAL INFORMATION: 1.4.1. Disputes: If any disputes or non-compliance with subject Performance Work Statement arise between either the contractor or Family Services Family Life/Readiness Coordinator, either party in concern must contact the Contracting officer for clarification, corrective action and resolution. 1.5. RESTRICTIONS: 1.5.1 The contractor is prohibited from receiving any moneys or to benefit from any source as a direct or indirect result of the services provided to Family Services clients. 1.5.2. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the Contracting Officer as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population. 1.6. ORGANIZATIONAL ACTIVITIES. If determined appropriate by the Commander, Contractor personnel may be invited to participate in organizational events (i.e., organization days, grand openings, etc.) at their place of duty in a paid status. Participation is subject to contractor approval. Contract personnel are not entitled to participate in morale and welfare activities and be paid by the government. The contractor can allow their employees to participate at company expense and have the contract amount reduced. 1.7. NON-CONFORMING SERVICES FORMULA. In conjunction with the Inspection of Services - Fixed Price clause, FAR 52.246-4 and Contract Terms and Conditions -- Commercial Items clause, FAR 52.212-4 the government reserves the right to make a monetary adjustment for non-conforming service and inadequate Quality Control. The Government shall utilize the following method for determining monetary adjustments: (a) Determine monthly line item cost per unit by using the bid schedule and or contractors pricing documents. (b) Determine the quantity of the service performed per month. (c) Determine the quantity of unacceptable service. (d) Divide the quantity of unacceptable service by quantity of service performed per month. (e) Multiply the percentage of unacceptable service by the line item cost per month for the monetary adjustment. 1.8. GOVERNMENT REMEDIES. The contracting officer shall follow FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Sep 2005), for contractor?s failure to perform satisfactory services or failure to correct non-conforming services. 1.9. PHYSICAL SECURITY. In the interest of installation security, all vehicles and personnel entering and operating on the installation are subject to search at any time. 1.10. SECURITY REQUIREMENTS. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by all security regulations of the installation. When any employee no longer works for the contractor at the installation, the contract manager shall return any government furnished identification card to quality assurance personnel or Security Forces. Contractors shall expect delays entering the base and entry and exit of restricted areas. If, for any reason, Security Forces or any military member detains a contractor or contract employee, the contractor is to comply with their request and is not to become combative or argumentative. Contractors will be required to undergo Antiterrorism Level I training within 30 days after contract award. 1.11. CONTRACTOR FURNISHED ITEMS AND SERVICES. Except for those items or services specifically cited in paragraph 3 to be Government-furnished, the Contractor shall furnish everything required to perform this contract in accordance with all of its terms. 1.12. EMPLOYEES. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106. The Contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours or deployment. The abrupt absence of these personnel shall not constitute an excuse for nonperformance under this contract. 1.13. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. Contractor access to the installation may be denied during a national emergency or crisis situation called by national command authorities, or overseas combatant commanders. Such circumstances are deemed beyond the government?s control and access to the installation may be limited in such circumstances on the basis of public safety and installation security. Should limited access be necessary, the responsible squadron will inform the contractor in writing and identify the date when services may resume, if possible. The contractor will not be held liable for lost time during these situations. 2. SERVICE SUMMARY Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. GOVERNMENT FURNISHED PROPERTY AND SERVICES: 3.1. The government will provide adequate storage and working space to include heat, lights ventilation, adequate current and outlets for use by the contractor. These facilities are located in building 3, RAF Fairford. 3.2. Sufficient telephones with appropriate DSN and commercial capabilities commensurate with fulfilling the responsibilities of this PWS. 3.3. Access to a Pentium computer with Microsoft Office software, a printer, and a dedicated e-mail account on the base LAN. 3.4. Supplies, forms, publications, reference materials from government or commercial sources directly related to Family Services. 3.5. Supplemental reference materials and training aids related to the Family Services Program procured from commercial sources. Requests for purchase of such material will be in accordance with applicable AF instructions and subject to budgetary constraints. 3.6. Access to copy machines/base reprographic facilities through the 420 ABS Orderly Room for reproduction of material directly related to Family Services. Contractor will comply with applicable policies/regulations concerning the quantities and types of material reproduced 3.7. Access to base conference and training rooms through the 420 ABS Command Section. 3.8. Key for access to Family Services Office. 3.9 Government will provide initial training as well as refresher training on an as needed basis to become knowledgeable about programs and services assigned to the Family Services Center. The Government will provide adequate funding to reimburse the contractor for reasonable travel, lodging, and per diem expenses to complete required training at RAF Mildenhall. Initial and refresher training courses are approximately five days in length. 4. APPENDICES NONE
