Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, FM442760880430, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-02 and Defense DCN 20050222. The solicitation listed here is 100% small business set aside. The Federal Supply Class (FSC) is 6515. The associated Standard Industrial Classification (SIC) is 3841. The North American Industry Classification System is 339112 and the associated business size standard is 500 employees. This request for quotation consists of the following: Epidural Infusion Pump System, CADD-Prizm PCS II OR EQUAL. CADD-Prizm PCS II Specifications: Size- 4.4cm x 10.4cm Weight- 568 grams or 20 oz. Power Sources- 9 volt alkaline or lithium battery (4 day capacity at standard rates), AC adapter Units- Milliliters, milligrams, micrograms Continuous rate- 0-30 ml/hr (or mg or mcg equivalent) PCEA Dose- 0-9.9 ml or equivalent Dose Lockout- 0-24 hours Delivery Limit- 0-1000ml in 1-2 hour increments Reservoir volume- 1-9,999 ml This systems includes- Item 0001: Epidural Infusion Pump- Yellow pump, model 6101, includes infusion pump with yellow keypad, pump pouch, 9-volt alkaline battery, pump key, remote dose card, carrying case, 2 extra operator's manuals, Smith?s Medical MD Part# 218861 OR EQUAL. Item 0002: Yellow Lock Box, Smith?s Medical MD Part# 216183 OR EQUAL. Item 0003: Lockable Pole Clamp, Smith?s Medical MD Part# 21-6120 OR EQUAL. Item 0004: Shipping charges for FOB Destination to Travis AFB, CA 94535 The following provisions and or clauses apply to this acquisition.
