May 22, 2006 STOLLER-NAVARRO JOINT VENTURE (SNJV) ? ADDENDUM 1 TO REQUEST FOR PROPOSAL (RFP) SN06R0088, BOREHOLE INSTALLATION ? DRILLING SERVICES, INDUSTRIAL SITES PROJECT, CAU 542 Addendum 1 to RFP SN06R0088 for Drilling Services at CAU 542 makes the following changes to the solicitation: 1. Questions and Answers to RFP SN06R008 are provided on www.Stoller-Navarro.com as Addendum 1. 2. Section G ? Contracts Data Requirements, is revised to delete the requirement for drillers? Nevada water well licenses. 3. The Statement of Work, entitled, ?Borehole Installation ? Drilling Services, Industrial Sites Project, Corrective Action Unit 542?, is hereby changed to delete the requirement for drillers? Nevada water well licenses. The following statement in the SOW is changed: Page 3, Section 2.1, paragraph 2. The sentence reading, ?The names of all licensed well drillers to be assigned to the project and a copy of their respective Nevada well-drilling licenses must be submitted with the bid.? is changed to read, ?The names of the lead driller and an alternate, if applicable, to be assigned to the project must be submitted with the bid.? Page 8, Section 3.1, paragraph 2, bullet 3. Delete the parenthetical statement ?(site specific, including Radiological Worker II)?. Bullet shall read only, ?OSHA 1910.120 8 hour refresher?. Page 5, Section 2.2, third paragraph. Delete this paragraph and replace it with the following: ?The preferred investigation method involves the drilling of boreholes ranging in size from an 8-in. to a 6-in. borehole at the final drilled depth. However, the surface cased interval will probably change to 9 ? inch if we have to telescope.? The Proposal Due Date remains May 30, 2006 at 4:00 pm, PDT.
