Description
This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. Solicitation Number W81K00-06-T-0250 is being issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquis ition Circular 97-27. CLIN 0001: One (1) Pediatric Patient Simulator Base Unit, one (1) Gas Accessory Kit-ECS H Cylinder, one (1) ECS Portability Battery System, two (2) training slots, and one (1) Moulage Trauma Kit; CLIN: 0002 One (1) Surgical SIM Base U nit, one (1) Surgical SIM Training Slot, Mfg: Medical Education Technologies, Inc. Solicitation provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, is hereby incorporated by reference. FAR Clause 52.212-2, Evaluation-Commercial Items. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR 52.211-6, if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation ) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Brooke Army Medical Center, Fort Sam Houston, TX 78234. FAR provision 52.212-1 [Instructions to Offerors  C ommercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability FAR 52.212-2 (compliance with the require ments of FAR Clause 52.211-6 if vendor offers and Equal Product), Price and Past Performance may be considered, with Technical Capability being significantly more important than Price. Salient characteristics of Pediatric Patient simulator. Functional pup ils that blink and can be adjusted for pupil dilation and constriction; simulator with automatic physiological and pharmacological responses, and drug recognition system. Salient characteristics of Surgical Sim Base Unit/Surgical Sim Training. Support mul ti-task and multi-user learning; ability to record learners performance ; measures and assess an individual learners progress. Past performance: The past performance questionnaire may be obtained as an attachment from the combined solicitation. Have the questionnaire sent directly back to this office by the point of contact who completed the form. The firm may fax the completed questionnaire to 210 916-3040 or 4698 (ATTN: Willie Copeland). If the offeror submit more than two past performance reference s, the Contracting Officer will consider only the first two listed references in the award decision. The Contracting Officer may also consider any additional past performance information that is available to the Government. The POC shall include contact information with the returned past performance questionnaire directly to the Great Plains Regional Contracting Office by the closing date. A copy of the past performance questionnaire may be obtained from the web site. Vendors shall include a completed c opy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation. FAR clause 52.212-4 [Contract Terms and Conditions  Commercial Items] applies to this acquisition to include the follow ing addenda: Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-7003  Electronic Submission of Payment Requests and DFARS 252.211-7003. Companies wishing to respond to this solicitation should provide this office with the following: a price quote, submitted on your company letterhead or your standard company quo tation forms, for the item showing solicitation number, unit price, extended price, and shipping charges, if any. Offerors must be signed. Each offeror shall include their Dun & Bradstreet (DUNS) Number and their commercial and Government Entity (CAGE) c ode.
