Description

The Veterans Integrated Service Network (VISN 6)/Centralized Acquisition Service (CAS) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to purchase three (3) trays of cardiac surgery micro ceramic instruments as described herein for the Department of Veterans Affairs Medical Center - Richmond, 1201 Broad Rock Blvd, Richmond, VA 23249. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2(a). The NAICS is 339112 with a small-business size standard is 500 employees. VISN 6/CAS intends to purchase three (3) trays containing the each of the following items (quantity is one (1) each per kit unless otherwise noted): Fehling Micro NH with lock (.5mm) length 180mm (KOD-9); Fehling Micro NH with lock (1.0mm) length 210mm (KOA-5); Fehling Micro NH with lock (1.0mm) length 230mm (KOA-7); Fehling Micro NH with lock (1.5mm) length 230mm (KOT-7); Fehling Micro Vascular Scissor 60 degree 180mm (BSK-3); Fehling Metz Scissor fine-curved 200mm (BOC-3); Fehling Vascular Scissor 25 degree 180mm (BOR-1); Fehling Vascular Scissor 125 degree 170mm (BOR-5); Fehling Vascular Scissor 90 degree 170mm (BOR-4); two (2) Fehling Micro Forceps (1.0mm) 210mm (COY-7); Fehling Sharp Straight Fine Metz (BOO-4); two (2) Fehling Gerald Forceps (1.0mm) 180mm (COK-1); Fehling Coronary Dilator (.5mm) 140mm (MNG-0); Fehling Coronary Dilator (1.0mm) 140mm (MNG-1); Fehling Coronary Dilator (1.5mm) 140mm (MNG-2); Fehling Coronary Dilator (2.0mm) 140mm (MNG-3); IMA Epicardial Retractor (Large); Fehling Sharp Curved Dissecting 180mm (BOO-1); two (2) Castaneda Suture Tag; Micro Scissors 3 ?? 60 degree (babies); Mayo Scissors curved 9? super cut; two (2) Resano Forceps 9?; Weary Nerve Hook, black finish, 7 ??; Dandy Nerve Hook 8 5/8?; and Jacobson Micro Needle holder titanium. Offeror shall include warranty information as appropriate with offer. Quotations shall be submitted on an all-or-none basis. Partial quotations shall be rejected. Quotations shall be valid 60 days from offer due date. Prices are F.O.B. Destination. All responses shall be for the specified manufacture/brand name or equal. Equal offers shall include: 1) descriptive literature and product number offered; 2) Separate unit cost of each item; 3) evidence of past performance of contracts of similar type, scope size and complexity that are ongoing or completed within the past three years, references with points of contacts must be identified, 4) delivery time. The following provisions and clauses apply to this solicitation: 52.212-1 - Instructions to Offerors-Commercial Items; 52.212-2 - Evaluation-Commercial Items; 52.212-4 -Contract Terms and Conditions-Commercial Items; 52.211-6 - Brand Name or Equal; 852.211-77 - Brand Name or Equal; 52.247-34 - F.O.B. Destination; and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items including clauses incorporated by reference under section b (5) (14-20) (23) (26) (32). In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at http://orca.bpn.gov/ in accordance with FAR 4.1201(a). This is a best-value procurement. Award will be made to the offeror whose offer conforming to the solicitation is determined to be most advantageous to the Government, price and other factors considered. Technical and past performance, when combined, are significantly more important than price. The Government intends to evaluate offers and award without discussion.
