Description
WORK STATEMENT UROLOGY Contractor will provide a board-certified physician in the diagnosis and treatment of Urological conditions. Services will be provided on site at the VA Medical Center, Manchester, NH two (2) clinic days per week. Clinic days are eight (8) hours each. The days of the week services shall be performed will be mutually agreed upon between the contractor and the Chief of Staff's office at the VA Medical Center, Manchester, NH The physician will be required to provide on site and telephone consultations for patients requiring Urology services as referred by their Primary Care and ER physicians. The intention is to treat and discharge and return to the care of the primary provider at the earliest appropriate time. Procedural services will include Outpatient Same Day Surgery. The average number of hours per OR time is established at four (4) hours, two (2) days per week. Procedures that may not be accomplished will be referred by the consultant to the appropriate VA Special training/experience in performing TUNAs. As part of care provided, contract physician is requested to prescribe for drugs within the formulary list of the Manchester VA facility whenever clinically appropriate, when needed non-formulary requests will be submitted by requesting provider. Estimated number of patients scheduled per clinic period range from 14 to 20 patients. All scheduling of appointments will be processed by the PSA's located in the Outpatient Clinic of the Manchester, VAMC. Contractor will hold privileges at local facilities Elliot, CMC and/or Parkland Medical Center for admitting during non clinic hours. On-call service 24/7 are required under the terms of this contract. The contract physician will be required to enter all reports for incorporation into the computer patient record system (CPRS) on the day of the visit and will satisfy VHA and medical center policies and JCAHO requirement for documentation, all notes will need to be signed within 7 days. The physician must possess computer skills to access CPRS utilized by the VA. Orientation to CPRS will be provided by the VA. Dictation service may also be used. Contractors will be given remote access to assist with completion of documentation in a timely manner. Contractors must maintain computer access by logging into the remote system at a minimum every 90 days. Services to be performed by the contractor will be performed within the VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. A contractor's employee must be present at the VA facility and must be actually performing the required services for the period specified in the contract, or the contract cost will be decreased accordingly during the billing cycle. Personnel assigned by the contactor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Personnel performing these services shall be board certified/eligible; BLS and ACLS certified and must meet the credentialing privileging requirements of the VA Medical Center, Manchester, NH. Services to be performed by the contractor will be under the direction of the Chief of Staff or his/her designee at the VA Medical Center, Manchester, NH. BLS is a requirement and will be the responsibility of the contractor to acquire and maintain. ACLS is required of any contractor that is requesting privileges to administer moderate sedation It is the responsibility of the contractor to have annual tuberculosis screening. New conversions to a positive PPD or new symptoms related to TB, if previously positive PPD, must be reported to the Manchester, NH VAMC Employee Health within 10 working days. It is the responsibility of the contractor to receive appropriate immunizations, including the Hepatitis B vaccine as indicated for health care workers with potential exposure to blood or body fluids. The Manchester, NH VAMC will provide the following: Scheduling of patients for Urology clinic, surgery and follow up. Access to the patients' electronic medical record. All laboratory tests, radiology examinations and other diagnostic tests. Support staff for Urology clinic and surgeries and referrals to other VA facilities. Necessary equipment for clinic and surgical procedures. 17. Any contractor that offers a group practice, it is requested that two or three providers service the VAMC. Additional providers may be added upon workload review and approval from the VAMC. New providers to the practice may be added only if deemed necessary and will be determined by the VAMC.
