Qty: 2, Bison 7, Power Unit Only, RAMVAC 826E0800.-- Qty: 2, 50-Gallon Cylindrical Tank used w/ Auto Wash @ Power Drain, RAMVAC 590053AWPD.-- Qty: 1, S1 Vacuum Stabilizer Control Kit, RAMVAC 8-600-426.-- Qty: 1, Remote Control Panel #1 (Air), RAMVAC 8-600-131.-- Qty: 1, Vacuum Switch ASSY. 0" to 30" HG, RAMVAC 003552.-- Qty: 1, Removal of existing vacuum system, hauling and disposal, electrical and plumbing for all services to existing and installation of new system. Work to be performed with clinic shut down on predetermined day and time and work completed in a timely manner. --- STATEMENT OF WORK FOR Installation of dental suction system in B-1040 1. DESCRIPTION OF WORK: a. Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, required to take out old high volume suction system and replace with a new high volume suction system. b. Location: Bldg 1040, Randolph AFB, Texas 2. PERSONNEL: a. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing, caps, hats, etc., bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. b. Contractor shall have certified personnel to accomplish work under this SOW. 3. QUALITY CONTROL: The contractor shall insure that all work performed under this Statement of Work (SOW) meets the quality established in paragraph 9 and 10. 4. PHYSICAL SECURITY: a. The contractor shall obtain vehicle passes for each vehicle used on Randolph AFB during the performance period. b. The Contractor shall be responsible for safeguarding all Government property if provided. At the close of each work period, facilities, equipment and materials shall be secured. 5. HOURS OF OPERATION: a. Normal Duty Hours: The contractor shall maintain the following services: 0730 (7:30 a.m.) to 1630 (4:30 p.m.), Monday through Friday. The contractor may be required to provide service on the weekend 0730-1630 to accomplish installation for this project. b. Recognized Holidays: The contractor is not required to provide service on the following days: New Year?s Day, January 1st Martin Luther King?s Birthday, 3rd Monday in January President?s Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veterans Day, November 11th Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th NOTE: Any of the above holidays falling on a Saturday will be observed on proceeding Friday. Holidays fallings on a Sunday will be observed on the following Monday. 6. INSPECTION AND ACCEPTANCE: The office having responsibility for inspection and acceptance of services is 12 CES/CEOE, Maintenance Engineering at (210) 652-3155. 7. GOVERNMENT FURNISHED PROPERTY AND SERVICES: a. Emergency Phone Numbers: (1) Medical Emergency: 911 (2) Security Police Phone Number: (210) 652-5700 (3) Fire Protection Phone Number: (210) 652-6915 8. CONTRACTOR FURNISHED ITEMS AND SERVICES: a. General: Except for those items or services specifically stated to be government furnished in section 7., the contractor shall furnish everything required to perform this Statement of Work. b. Equipment: The contractor shall furnish equipment that is required for the complete installation of a high volume dental suction system. c. Transportation: The contractor shall furnish own transportation as needed to accomplish the requirements of this statement of work (SOW) and insure each vehicle has the proper insurance, registration that is required by law, and each person has proper drivers license. 9. SPECIFIC TASKS: a. The contractor shall dismantle/remove the existing high volume suction unit in room 1B205 of B-1040. b. The contractor shall provide all necessary equipment to install a new high volume suction system to include all electrical and plumbing connections. c. The contractor shall disconnect all electrical/plumbing components that may be needed for dismantling/removing of equipment. d. The contractor shall keep area clean after the days work e. The contractor shall use safety equipment at all times while performing task 10. SAFETY: a. The contractor shall call the fire department to be issued a fire permit when welding/soldering/brazing or any open flame is to be used and shall have a fire distinguisher present at all times b. The contractor shall be sure that all employees follow all safety procedures while working on this project and have all safety equipment available to meet safety requirements. 11. LICENCES: Contractor shall and will be responsible for all licenses (if needed) that are required by state/federal/base to complete this task. 12. INSPECTION AND SITE TOUR: Government representative from 12 CES/CEOE, will conduct daily inspection on the progress of the instillation of equipment to ensure SOW and safety is followed. 13.FINAL INSPECTION: Government representative (12 CES/CEOE 652-3155) will make sure that the work is in compliance with all regulations, SOW, and customer satisfaction. 14. PERIOD OF PERFORMANCE: The contractor will coordinate with the medical facilities manager (Mr. Mata, 652-2521) to set up the date of installation when he accepts the contract.
