- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request For Quote (RFQ) reference number W912L2-06-T-1010. This solicitation document and incorporated provisions and clauses are th ose in effect through Federal Acquisition Circular 2005-09. This procurement is being issued as a 100% Total Small Business Set-aside. It is anticipated that a single Blanket Purchase Agreement (BPA) will be awarded for a sixty (60) month period. FAR 52. 212-2, Evaluation of Commercial Items, apply. The following factors shall be used to evaluate offers: (1) Technically acceptable, (2) Price. A single BPA will be made to the lowest cost, technically acceptable responsible offeror. Any BPA call awarded u nder this BPA is guaranteed a minimum order award of $500.000 during the life of the BPA. Cumulative total amount of calls shall not exceed $2,000,000.00. The Government reserves the right to make a technical acceptability determination without discussion with the offeror. Offeror shall price all CLINs. In the event the unit price(s) and extended pricing are ambiguous, the government shall use the indicated unit price(s) for evaluation and award purposes. Offerors shall include with the offer a complete d copy of the provision in FAR 52.212-3, Offeror Representations and Certifications. Contractor shall provide all clinical equipment, clinical supplies, dentists, dental assistants, hygienists, x-ray equipment, x-ray technicians, digital X-ray systems, intra-oral cameras, ultra-sonic cleaning scalers, and any other required material in a mobile 2 or 3 operatory unit to provide on-site dental examinations and routine dental care to soldiers in preparation of deployment. More than 1 mobile operatory may be required at one time. In order to be considered responsive the contractor shall describe in full detail theie capabilities to provide a completely mobile unit or units. No interior office space will be provided by the Government. The only provisions to be made the contractor are: parking space and a power supply. Contractor must provide all dental care in their own mobile unit(s). The examinations and dental care shall be provided mainly in Phoenix, Marana and Casa Grande, Arizona, but may be neede d statewide. Contractor shall allow a minimum of 10 hours each day to accommodate for the exams during the hours of 0600-1600. All deploying soldiers must have a complete dental evaluation regardless of prior examinations. A complete exam shall in clude 4 bitewing radiographs, panographs older than 10 years and a health history and treatment plan placed into the Army Denclass web database. Soldiers identified as class 3 should receive immediate care to place them into a class 2 or 1 category. R estorative costs for returning soldiers to class 2 or 1 exceeding $800.00 require approval of Chief Dental Officer and the Contracting Officer. Clinical evaluation shall be by a licensed dentist. Each exam will be entered into the DENCLASS reporting syst em and annotated on the SF 603 Health Record and the SF 5570 Health Questionnaire for Dental Treatment, of the Soldiers dental records. Personnel performing the dental screening examination have a moral and ethical obligation to inform the Soldier if the y observe or are apprised of any sign or symptom for which the Soldier should obtain further evaluation or care. Soldier Readiness Processing for Mobilizations will include treatments for those identified as Class III. Quotes shall include separate line items including: 1. Costs for initial examination including 4 bite wings, 2. panographs, and 3. a schedule of fees associated with procedures necessary to u pgrade soldiers from Class 4 and Class 3 to Class 1 or Class 2. Procedures that require treatment that can not safely be provided in a mobile setting shall not be provided for during the period.
