Description
The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to upgrade Gastroenterology Ultrasound equipment at the VA Medical Center San Francisco, 4150 Clement Street, San Francisco, CA 94121 This is a Department of Veterans Affairs facility within the Veterans Integrated Service Network (VISN 21). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. The NAICS is 339112. VISN 21 CCA intends to solicit quotations and purchase the following items for theVA Medical Center San Francisco: 1) Three (3) Video Processor w/high def and NBI compatibility, electronic magnification, picture-in-picture, structural enhancement, digital capture w/annotation, auto iris, surgical product compatibility and standard accessories; 2) Three (3) Xenon light source with coated filter for NBI, Surgical product compatibility and standard accessories; 3) One (1) Therapeutic video gastroscope with 11.0mm diameter, 3.7mm channel, 120 degree field of view, 103cm working length, and angulation of 210/90 degree (up/down), 100/100 degree (right/left). Complete with standard accessories; 4) One (1) Therapeutic Video Duodenoscope w/v - lock elevator; 5) Two (2) Video Gastroscope w/wide screen image, and 8.8mm diameter, 2.8mm channel, 140 degree field of view, 103 cm working length and angulation of 210/90 degree (up/down), 100/100 degree (right/left). Includes biopsy forces (2), mouthpiece (2) complete with standard accessories; 6) Four (4) Wide-angle view, innoflexTM adjustable stiffness video colonscope with wide screen image, 12.8mm diameter, 3.7mm channel, 170 degree field of view, 168cm working length and angulation of 180/180 degree (up/down), 160/160 degree (right/left). Includes biopsy forceps (2), auxiliary water tube (1), auxiliary water inlet cap (1), complete with standard accessories; 7) InnoflexTM adjustable stiffness ultra-slim video colonoscope with wide screen image and 11.3mm diameter, 3.2 channel, 140 degree field of view, 168cm working length, and angulation of 180/180 degree (up/down) , 160/160 degree (right/left)., includes biopsy forceps (2), Auxiliary water inlet cap (1), complete with standard accessories; 8) One (1) High definition video gastroscope with NBI compatibility, close focus, 9.8mm diameter, 2.8mm channel, 140 degree field of view, 103cm working length, angulation of 210/90 degree up down, 110/110 degree right/left and standard accessories; 9) One (1) High definition colonscope with NBI compatibility, wide angle viewing innoflexTM adjustable stiffness, close focus, 12.8mm diameter, 3.7mm channel, 170 degree field of view 168cm working length, angulation of 180/180 degree up/downand 160/160 degree left/right and standard accessories; 10) One (1) Orea-Radial electronic scope; 11) One (1) RAE: Upgrade kit; 12) One (1) NDS 19" Radiance 24 vdc sith SDI, DVI-I, RGBS, Y/C, COMP, SOG; 13) One (1) Wall mount for LCD flat panel monitors. Offeror shall include warranty information with offer. Quotations shall be submitted on an all-or-none basis. Partial quotations shall be rejected. Quotations shall be valid 60 days from offer due date. Prices are F.O.B. Destination. All responses shall be for specified manufacture/brand name or equal. 1) descriptive literature and product number for time offered; 2) Separate unit cost of each item; 3) evidence of past performance of contracts of similar type, scope size and complexity that are ongoing or completed within the past three years, references with points of contacts must be identified. 4) Delivery time. This is a best-value procurement. Award will be made to the offeror whose offer conforming to the solicitation is determined to be most advantageous to the Government, price and other factors considered. The Government intends to evaluate offers and award without discussion. However, the Government reserves the right to conduct discussions later if determined necessary by the Contracting Officer.
