Description
This is a combined synopsis/solicitation for commercial items in accordance with the format in Federal Acquisition (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation of Commercial Items as supplemented with additional information included in this announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. RFQ60902-036-06 is issued as a request for quotation. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-09. The North American industrial Classification System Code is 621999. The small business standard is $9 Million. This solicitation is 100% set-aside for small business. The Government intends to make a single award of a firm fixed price non-personal services contract. The resultant contract will not constitute a employee/employer relationship. Award will be made to the quoter who submits the most advantageous quote to the Government based on price, technical capability, and past performance. GENERAL REQUIREMENTS: The Federal Bureau of Prisons (BOP), Federal Correctional Institution (FCI), located at 1529 W. U.S. Hwy 366, Safford, Arizona 85546 has a requirement for a Radiological Technician for the period of a base year with four (4) twelve (12) month option periods for renewal. The office listed above intends to make a single award of a firm fixed price type contract. Award is estimated to be on or about July 10, 2006, allowing 21 days from the date of award to begin performance. REQUIRED SERVICES: The successful contractor shall be capable of providing Radiology Technician services as follows: Performs all types of radiographs (chest, skull, joints, extremities, etc.) Exercises extreme tact and patience in positioning and instructing patient and in obtaining the cooperation and understanding of the patient. Gauges the thickness of the body part to be X-rayed; inserts cassette into stand; gauges focal spot distance; sets exposure time and controls according to density required; and operates panel switch. Uses proper protective measure to avoid injury to patient and self through radiation, wears radiation check badge. Maintains necessary records, unpacks, and stores films and developing, fixing and replenishing solutions. Refers to procedure manuals and radiology books to ensure proper procedures and techniques are used, that minimal radiation is conveyed to patients, and that proper precautions of radiation protection are used. Keeps daily records, including date, patient's name, registration number, technique used, and part of body x-rayed, whether OP or IP, ensures requisitions are filled out properly; fills out monthly and annual reports. Cleans cassettes, keeping them free of dust, blood and debris which may cause artifacts which interfere with correct readings of films; ensures maximum protection of unexposed films and maintains a clean and orderly dark room. Reads X-rays sufficiently to ensure that film is artifact free, correctly positioned and exposed, correctly developed and is best film that can be taken; and takes initiative to take additional films to obtain clearer diagnosis. Calls significant findings to attention of medical officer. Reads reports on X-rays made by radiologist for his comments and possible suggestions for additional positions and techniques that will aid in further diagnosis. Knows where films are sent to be read for proper procedure for mailing. Mixes and changes developing solutions with right proportions and determines when they need changing by film observation and technical knowledge. Can take any position or technique requested on any part or bone in the body. ROUTINE SUPPLIES: In recognition of institution security concerns, routine supplies and equipment that the contractor may need while at the institution will be furnished by the Government. If the contractor requires special equipment or supplies a written request may be submitted to the Health Services Administrator for consideration. Additionally, the Government will furnish all required Government forms to be used in the performance of services. All records maintained by the contractor under this contract will remain the property of the U.S. Government, and shall at no point be removed from the premises of the Institution with prior approval from the Warden. The resultant contract may require the contractor to operate a system of records and will require adherence to the Privacy Act of 1974. PLACE OF PERFORMANCE: FCI Safford is a low security facility housing approximately 800 male inmates with the population growing to approximately 1300 male inmates by 2008. All services will be provide within the confines of the Federal Prison listed below: FCI Safford Health Services Department 1529 W. U.S. Hwy 366 Safford, AZ 85546 MINIMUM QUALIFICATIONS: Contract personnel performing Radiology Technologist/Technician work must be a high school graduate. Contract personnel must possess a current licence or certification as a Radiology Technologist/Technician. Must have a formal course of study in Radiological Technology. The contractor shall provide with their the name of the candidate(s) that will be providing services to FCI Safford and photocopies of documents evidencing their own and/or their candidate(s) ability to meet qualifications to perform required services as stated in the SOW. Examples of documentary evidence include a copy of high school diploma or equivalent, letters of recommendation, licenses, certifications, resumes, etc. PERFORMANCE SCHEDULE: The contractor will perform one(1) four(4) hour session each week. This weekly session will be scheduled in advance and with the Hospital Service Administrator?s approval. Sessions will be scheduled between the hours of 7:30a.m. and 3:00p.m., Monday through Friday, excluding Federal holidays. Due to the nature of the correctional setting, delays in performance of the work may be expected. Such delays may include but are not limited to inclement weather, electrical or mechanical problems, institution lock down, or other security matters which are beyond the Governments control. When contract employees report to the institution but are unable to perform services due to such delays, the contractor will be paid for one hour of the session. Normally, any remaining sessions for theat day will be suspended, the contractor will be required to leave and the contractor will not be paid for the remaining unformed sessions. All efforts will be maid by the Government to inform the contractor prior to the contractors work day. If the contractor has been notified in advance of reporting to work the Government will not be liable for any lost sessions. RESTRICTIONS ON CONTRACT WORKERS: The contractor shall not supervise inmates, supervise Bureau of Prisons staff, participate in inmate accountability, requisition, sign for or pick up supplies, formulate policy or procedures, attend meetings, staff recalls or staff functions. INMATE MANAGEMENT: The provision of services under this proposed contract will require frequent and unsupervised contact with inmates. The contractor will not be responsible for the management or inmates except to ensure that those inmates involved in activities within the contractor?s scope of work shall abide by all rules, in effect, to ensure the safety and well being of all participants. The contractor shall report any infractions of rules and regulations by any inmate immediately to the COTR. INSTITUTION SECURITY: The contractor agrees to adhere to all regulations prescribed by the institution in regards to the safety, custody, and conduct of inmates. The Bureau of Prisons requires all non-employees to sign in and out of the institution for security reasons. Additionally, the contractor will be required to wear an ID badge at all times. The ID badge will be provided by the Government. All contract personnel providing services within the confines of the Federal Correctional Institution shall have a complete background investigation conducted in accordance with BOP Program Statement 3000.2, ?Personnel Manuel?. The following requirements apply to those individuals delivering services inside the perimeter of the Institution: 1. National Crime Information Center(NCIC) 2. Name Check 3. FD-258 Fingerprint Check 4. Law Enforcement Agency Check 5. Vouchering of Employers over the past five years 6. Complete SF 171, Application for Federal Employment 7. Release of Information By signing the contract documents, the contractor hereby agrees to complete the required documents and undergo the listed procedures. Any individual who does not pass the security clearances will be not able to work within the secure perimeter of the institution. The AHSA will process the initial investigative requirements within ten calender days from the date the contractor submits a properly completed background package. The final determination and completion of the security investigation procedures will be made at the sole discretion of the Bureau of Prisons. Once a contractor employee clears the initial screening requirements, a urinalysis test will be required before the individual is allowed access into the institution. If the results of the urinalysis test are positive, that individual clears both the initial screening requirements and the urinalysis test, that individual will be issued a pass for access into the institution. In addition, each contractor employee must attend a four-hour orientation program at the institution. The purpose of this program is to familiarize contract employees with the BOP operations, the institution, and general rules of conduct and procedures inside the institution. Contractor employees shall adhere to all institution regulations regarding conduct and performance. Contractor employees will be allowed access to the institution at the sole discretion of the Chief Executive Officer (CEO). Any individual performing under this contract may be removed if it becomes apparent that his or her conduct does not reflect the conduct of that prescribed for those people performing under non-personal contracts. Primary concerns are the amount of contract that may occur between the contractor?s employees and the inmate population during the performance of this contract. Notice of Contraband: The contractor will inform all his/her employees that intoxicating beverages, drugs, weapons, etc., will not be allowed on the institution property in any vehicle or upon any person. Any person suspected of utilizing intoxicating beverages, drugs, weapons, etc., in the performance of this contract will not be allowed on the premises of this institution. Tool boxes will be kept locked at all times except for removal of needed tools, and these shall be in constant surveillance of person utilizing the tools. Introduction of contraband shall be subject to penalties prescribed by 18 USC 1791. The use of explosives is absolutely forbidden. METHOD OF PERFORMANCE: The contractor has sole responsibility and discretion for choosing and implementing the manner and method of performing tasks outlined in this SOW as long as the requirements are met in accordance with the policies described within. The services required are for a non-personal health care services type contract, as defined in the Federal Acquisition Regulations (FAR), Part 37.101, under which the contractor is an independent contractor. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical and/or professional aspects of services rendered (i.e., professional judgements, diagnosis for specific medical treatment). The contractor is required to indemnify the Government for any liability producing act or omission by the contractor, its employees, and agents occurring during contract performance. The contractor is required to ensure that all subcontracts for provision of health care services contain the FAR clause 52.237-7, including the maintenance of medical liability insurance. These services, although not directly supervised, will be under the administrative review of the Health Services Administrator and/or the designated Contracting Officers Technical Representative (COTR). PAYMENT TERMS: It is the preference of the FCI to be billed on a monthly basis. Invoices should be submitted in accordance with FAR 52.212-4(g) and payment will be rendered in accordance with the Prompt Payment Act (31 U.S.C. 3903). The contractor may offer discount payment terms for prompt payments as specified in Block 12 on the SF-1449 of this solicitation. CONTRACT PRICING: Contract pricing shall include all charges to the Government for performing the services required by this solicitation/contact (e.g., bonding, overhead, profit, supervision, minimum wages, etc.). the Government reserves the right to award without discussion. Therefore, the initial quote should contain the quoters best terms for a cost or price standpoint. Quoters are hereby notified that an offer of contract award shall be extended to the responsive/responsible quoter submitting the lowest priced, technically accepted quote. PRICING METHODOLOGY: The quantity of services to be provided by the Contractor is estimated below and are not a representation to a quoter or contractor that eh estimated quantities will be required aor ordered aor that conditions affecting requirements will be stable or normal. Pursuant to FAR 17.203(b), the Government?s evaluation shall be inclusive of options. Pursuant to FAR 17.203(d), the quoter may offer varying prices for options, depending on the quantities actually ordered ant the dates when ordered. Contract pricing shall be based upon a per session basis. Please note that there will be processing time required to enter the institution that should be reflected in the quoted. Each session is defined as one(1) four hour period. If the contractor is turned away at the institution due to a delay in performance as stated in performance schedule paragraph of this solicitation the price quoted for one session will be divided by four(4) to adjust payment for performance. The Contractor shall commence full performance of services under this contract with in 21 calendar days from effective date of award of the contract. In accordance with FAR 46.503 and 46.403(a)(6), the place of acceptance for services under this contract is at the destination, FCI Safford, located in Safford, Arizona. Ordering Official: Task orders for service may be issued only by the Contracting Officer, Administrative Contracting Officer, or an ordering official for the institution with an appropriate certificate of appointment. NON-PERSONAL SERVICE STATUS: Award of this requirement will result in a contractual arrangement only and shall not be construed as a personnel appointment with the BOP. The services shall not be constitute an employer/employee relationship. Payments to the Contractor shall be based on the provision of an end product or the accomplishment of a specific task. Results to be obtained are within the Contractor?s own unsupervised determination. The Contractor will not be subject to Government supervision but its efforts will be monitored for quality assurance.
