- The Department of Veterans Affairs VISN 9 Acquisition Service Center, 3400 Lebanon Pike, Murfreesboro, TN 37129 is soliciting on behalf of the VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104 for Contractor to provide Gastroenterology/Colonoscopy services to veteran beneficiaries in multiple locations within the Memphis VA Medical Center service area. Contractor shall provide services at a location in the following geographical areas: Memphis, Tennessee City limits; Dyer, Gibson, Lauderdale, Haywood, Obion, and Weakley, Tennessee counties; Craighead, Clay, Greene, Mississippi, Randolph, Lawrence, and Poinsett, Arkansas counties; Lee County, Arkansas, and Chickasaw, Itawamba, Prentiss, Union, Pontotoc, Calhoun, Tishomingo, and Tippah Mississippi counties; and Chester, Crockett, Madison and Hardin, Tennessee counties. Contractor shall provide all equipment; supplies and staff necessary to perform, implement and administer Gastroenterology services for veterans of the VA Medical Center, Memphis, TN in accordance with all terms, conditions, provisions, schedules and specifications of this solicitation. Contractor shall provide colonoscopies, inclusive of CPT codes 45378-45385 and attendant procedures (CPT code 45330), as clinically required. Contractor must be located within a 100 mile radius of the VA Medical Center, Memphis, TN, providing services in one or more locations as detailed in the Schedule of Services. Contractors do not have to provide an offer for each community / site. Although this may be a multiple award contract, the contractor that receives an award will be the sole recipient for that particular geographic location. Low Risk background investigations shall be conducted on all Contractor and Subcontractor personnel. The cost of these investigations shall be borne by the Contractor. This solicitation for commercial items will be conducted in accordance with FAR Part 13.5 test program with the procurement conducted in accordance with FAR Part 12, in accordance with IL 90-99-4. Under the Authority of Public Law 1040-262 and 38 USC 8153, the Contractor agrees to provide Health Care Resources in accordance with the terms and conditions stated in the solicitation. The North American Industry Classification System Code (NAICS) is 621111 and the small business size standard is $9 Million. Solicitation will be available on or about June 30, 2006, with offers due on or about July 30, 2006. The contract period will be one year from the date of award, with the Government having the option to extend the contract for one (1) additional one-year period. Hard copy solicitations will not be made available. No telephone calls are accepted. Contractors are advised that they are responsible for obtaining any amendments to the solicitation. All offers must be returned to the address in block 9 of Standard Form 1449.
